支气管镜下局部治疗联合全身抗结核治疗支气管结核疗效分析

Analysis on efficacy of local treatment under bronchoscope combined with systemic antituberculosis therapy in the treatment of bronchial tuberculosis

目的探究支气管镜下局部治疗联合全身抗结核治疗支气管结核的临床疗效。方法选取我院2017年5月~2018年5月收诊的64例支气管结核患者作为研究对象,并按照随机原则将所有患者分为对照组(n=32)与观察组(n=32)。对照组给予全身抗结核治疗,观察组则在对照组的基础上联合支气管镜下局部治疗。观察比较两组临床疗效、痰菌转阴率,同时比较两组胸部CT显示病灶吸收率及肺不张率。结果观察组的治疗总有效率(93.75%)、痰菌转阴率(96.88%)、病灶吸收率(90.63%),明显高于对照组的(75.00%)、(71.88%)、(68.75%);而观察组的肺不张率(6.25%)明显低于对照组(25.00%),差异有统计学意义(P <0.05)。结论基于全身抗结核治疗基础上联合支气管镜下局部治疗能有效提高支气管结核患者的临床效果和痰菌转阴率,有利于促进病灶及时吸收,降低肺不张率,临床价值较高,值得推广应用。

Objective To investigate the clinical efficacy of local treatment under bronchoscope combined with systemic antituberculosis therapy in the treatment of bronchial tuberculosis. Methods 64 patients with bronchial tuberculosis admitted in our hospital from May 2017 to May 2018 were selected as the observation subjects,and all patients were randomly divided into the control group(n=32) and the observation group(n=32).The control group was given systemic antituberculosis therapy,while the observation group was combined with local treatment under bronchoscope on the basis of control group.The clinical efficacy and sputum negative conversion ratio of the two groups were observed and compared,while the lesion absorptivity and atelectasis rate displayed by chest CT of the two groups were compared. Results The overall response rate(93.75%),sputum negative conversion ratio(96.88%),and lesion absorptivity(90.63%) in the observation group were significantly higher than those in the control group(75.00%,71.88%,68.75%);While the atelectasis rate(6.25%) in the observation group was significantly lower than that in the control group(25.00%)(P < 0.05). Conclusion The combination of local treatment under bronchoscope on the basis of systemic antituberculosis therapy can effectively improve the clinical effect and sputum negative conversion ratio of patients with bronchial tuberculosis,promote the timely absorption of lesions,reduce the atelectasis rate,with a high clinical value,which is worthy of promotion and application.
