摘要
目的探讨3D打印在骨科复杂Pilon骨折的临床应用效果。方法选取我院2014年5月~2018年5月所收治的92例Pilon骨折手术患者,随机分组经伦理委员会通过以后,按照随机数字表法分为观察组和对照组,其中观察组患者46例,采用3D打印进行患者手术治疗,对照组患者46例,采用传统切开复位钢板内固定手术进行患者治疗,通过两组患者并发症概率以及治疗13周后踝关节Mazur功能性的评分进行比较。结果观察组利用3D打印,所用手术时间以及患者出血量都低于对照组,差异有统计学意义(P<0.05);观察组的治疗效果高于对照组45.65%,其中观察组的优良率为95.65%(P<0.05);两组患者术后出现伤口感染、伤口愈合时间进行比较,对照组病发率(26.1%)稍高于观察组(19.6%),差异无统计学意义(P>0.05)。结论在骨科复杂Pilon骨折治疗中,运用3D打印技术进行手术治疗,观察组患者术后并发症发生概率明显降低,观察组患者踝关节功能的康复情况,与对照组传统手术切开复位钢板内固定术后恢复情况相比较,有明显的优势,在患者术后的伤口愈合情况以及伤口感染两组无太大差异。因此,3D打印在骨科复杂Pilon骨折手术治疗中的应用效果显著,值得临床推广。
Objective To explore clinical application effect of 3D printing in complex Pilon fracture in department of orthopedics.Methods 92 patients with Pilon fractures who were treated and underwent surgery in our hospital from May 2014 to May 2018 were selected.After being approved by the ethics committee,they were divided into the observation group and the control group according to the random number table method.46 patients in the observation group underwent surgery by 3D printing while 46 patients in the control group underwent traditional open reduction and plate internal fixation.Complication rates and ankle joint Mazur functional scores were compared between the two groups after 13 weeks of treatment.Results The observation group used 3D printing,the operation time and the bold loss were lower than those of the control group,the difference was statistically significant(P<0.05).The excellent and good rate of the observation group was 95.65%,higher than 45.65%in the control group(P<0.05).The wound infection and wound healing time after surgery were compared between the two groups.The incidence of the control group(26.1%)was slightly higher than that of the observation group(19.6%),the difference was not statistically significant(P>0.05).Conclusion In treatment of complex Pilon fractures in department of orthopedics,3D printing technology is used for surgical treatment,and the incidence of postoperative complications is significantly reduced in the observation group.The rehabilitation of ankle joint function in the observation group is significantly better than that in the control group after conventional surgical open reduction and internal fixation.There is no significant difference in wound healing and wound infection between the two groups.Therefore,the application of 3D printing in the surgical treatment of orthopedic complex Pilon fractures is significant and worthy of clinical promotion.
