产前护理在改善产妇瘢痕子宫阴道分娩结局中的影响分析

Exploration on effects of prenatal care on improving outcomes of vaginal delivery in parturient women with scarred uterus

目的分析产前护理在改善产妇瘢痕子宫阴道分娩结局中的影响。方法将在我院妇产科2018年1~12月收治的瘢痕子宫拟阴道分娩待产妇300例,随机分成对照组(150例)与实验组(150例),其中对照组产妇采取常规护理,而实验组产妇采取产前护理干预。比较两组产妇护理前后负性情绪评分、各产程时间、经阴道分娩成功率、产后出血、新生儿窘迫率。结果护理后实验组SDS、SAS评分明显少于对照组,实验组产妇第一、二、三产程时间、产后出血量均明显少于对照组,Apgar评分高于对照组,差异有统计学意义(P<0.01);实验组与对照组产妇转剖宫产率(6.00%、15.33%),阴道分娩率(94.00%、84.67%),出血率(0.67%、11.33%),胎儿窘迫率(1.33%、12.67%)比较,差异有统计学意义(P<0.01)。结论在产妇瘢痕子宫阴道分娩中,采取产前护理干预,可明显减轻产妇产前负性情绪,缩短产妇各个产程时间,提高经阴道分娩成功率,减少产后出血及新生儿窘迫发生率。

Objective To analyze effects of prenatal care on improving the outcomes of vaginal delivery in parturient women with scarred uterus.Methods Three hundred pregnant women with scarred uterus who were treated in department of obstetrics and gynecology in our hospital and planned to undergo vaginal delivery recently(from January to December 2018)were selected.According to the random number table method,they were divided into the control group(150 cases)and the experimental group(150 cases).Parturient women in the control group were given routine nursing while parturient women in the experimental group were given prenatal care intervention.Negative emotional score before and after nursing,time of delivery,success rate of vaginal delivery,postpartum hemorrhage and neonatal distress rate were compared between the two groups.Results The SDS and SAS scores of the experimental group after nursing were significantly lower than those of the control group.The time for the first,second and third stage of labor and the amount of postpartum hemorrhage in the experimental group were significantly less than those in the control group,and the Apgar score was higher than the control group,and the difference was statistically significant(P<0.01).Conversion to cesarean delivery rate(6.00%,15.33%),vaginal delivery rate(94.00%,84.67%),bleeding rate(0.67%,11.33%)and fetal distress rate(1.33%,12.67%)in the experimental group and the control group had statistically significant difference(P<0.01).Conclusion In the vaginal delivery of maternal scar uterus,prenatal care intervention can significantly reduce the prenatal negative emotions of parturient women,shorten the maternal labor time,improve the success rate of vaginal delivery,reduce the incidence of postpartum hemorrhage and neonatal distress.
