胫骨高位截骨术治疗膝内侧间室骨关节炎的临床疗效分析

Clinical efficacy analysis of high tibial osteotomy in the treatment of knee medial compartment osteoarthritis

目的探讨胫骨高位截骨术治疗膝内侧间室骨关节炎的临床疗效。方法选取2017年1~12月在医院接受诊断治疗的膝内侧间室骨性关节炎患者60例为研究对象,随机分为研究组与对照组,各30例。对照组采用关节镜下膝关节清理术治疗,研究组采用胫骨高位截骨术治疗。比较两组治疗后JOA与Lysholm评分。结果术后4周,两组JOA评分较术前显著改善,差异有统计学意义(P<0.05);术后4周,研究组JOA评分显著高于对照组,差异有统计学意义(P<0.05)。两组术后6个月Lysholm总分均显著提高,与术前比较,差异有统计学意义(P<0.05);术后6个月,研究组Lysholm总分显著高于对照组,差异有统计学意义(P<0.05)。结论胫骨截骨术治疗骨性关节炎能显著改善膝关节功能,缓解疼痛症状,术后力线改善满意。

Objective To investigate the clinical efficacy of high tibial osteotomy in the treatment of knee medial compartment osteoarthritis.Methods 60 patients with knee medial compartment osteoarthritis diagnosed and treated in the hospital from January 2017 to December 2017 were selected as subjects of this study.The patients were randomly divided into study group and control group,with 30 patients in each group.The control group was treated with arthroscopic debridement of knee joint,and the study group was treated with high tibial osteotomy.The JOA and Lysholm scores after treatment were compared between the two groups.Results The JOA score was significantly improved at 4 weeks after surgery compared with that before surgery,and the difference was statistically significant(P<0.05).The JOA score of the study group at 4 weeks after surgery was significantly higher than that of the control group,and the difference was statistically significant(P<0.05).The total Lysholm scores at 6 months after surgery were significantly improved in both groups,and the difference was statistically significant(P<0.05).The total Lysholm scores of the study group at 6 months after surgery was significantly higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion The treatment of osteoarthritis with tibial osteotomy can significantly improve knee joint function,relieve pain symptoms,and satisfactorily improve the postoperative force line.
