助产士主导视频教学及模拟分娩对初产妇分娩疼痛及结局的影响 被引量:5

摘要 目的探讨助产士主导视频教学及模拟分娩对初产妇分娩疼痛及结局的影响。方法选取信阳市息县人民医院106例初产妇为研究对象,采用随机数字表法随机分为观察组和对照组,每组53例。对照组产妇行常规产前检查与宣教,观察组在此基础上实施助产士主导视频教学及模拟分娩。观察对比两组产妇分娩过程中应激程度[汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)],第一产程、第二产程、第三产程时间,第一产程活跃期疼痛程度以及分娩结局。结果观察组产妇分娩中焦虑、抑郁程度明显轻于对照组,差异有统计学意义(P<0.05);观察组产妇第一产程、第二产程时间短于对照组(P<0.05),第三产程时间差异无统计学意义(P>0.05);观察组产妇第一产程活跃期疼痛程度低于对照组,差异有统计学意义(P<0.05);观察组产妇阴道助产、自愿剖宫产、非自愿剖宫产比例低于对照组,正常阴道分娩比例高于对照组,差异均有统计学意义(P<0.05)。结论初产产妇应用助产士主导视频教学及模拟分娩可缩短产程,减轻产妇身心痛苦,改善分娩结局。 Objective To investigate the effects of midwife-led video teaching and delivery simulation on the delivery pain and outcomes of primiparas.Methods A total of 106 primiparas in our hospital were selected as subjects.They were randomly divided into the observation group and the control group by random number table method,with 53 cases in each group.The control group was given routine prenatal examination and health education,while the observation group was given midwife-led video teaching and delivery simulation.The stress level during delivery[using self-rating anxiety scale(SAS),self-rating depression scale(SDS)],time of first stage of labor,second stage of labor,and third stage of labor,pain degree in active first stage of labor and delivery outcomes were observed and compared between the two groups.Results The degree of anxiety and depression during delivery in the observation group were significantly lighter than those in the control group(P<0.05).The time first stage of labor and second stage of labor in the observation group were shorter than those in the control group(P<0.05),and there was no significant difference in the time of third stage of labor(P>0.05).The pain degree in active first stage of labor in the observation group was lower than that in the control group(P<0.05).The proportions of vaginal midwifery,voluntary cesarean section and involuntary cesarean section in observation group were lower than those in the control group while the proportion of normal vaginal delivery was higher than that in control group(P<0.05).Conclusion Midwife-led video teaching and delivery for primiparas can shorten the labor process,reduce physical and mental pain,and improve the delivery outcomes.
