“动态针尖定位”桡动脉穿刺在规范化培训中的应用

Application of "Dynamic Needle Tip Positioning" in Radial Arterial Catheterization in Standardized Training

目的探讨超声引导"动态针尖定位"行小儿桡动脉穿刺置管在麻醉规范化培训教学中的效果。方法选取麻醉专业规范化培训住院医师20人,随机分为超声组(A组)和传统教学组(B组),每组10人。A组采用超声引导桡动脉穿刺置管,B组采用传统的盲穿法。结果 A组第一次穿刺成功率30.0%,B组第一次穿刺成功率12.0%(P=0.027);A组总成功率44.0%,B组总成功率24.0%(P=0.035)。A组100.0%的规培医生对学习超声引导穿刺有极高的学习兴趣,并认为超声技术是麻醉医生必不可少的技能。结论超声引导"动态针尖定位"行小儿桡动脉穿刺置管教学有利于提高初学者成功率,增强操作信心,提高学习兴趣。

Objective To investigate the application of ultrasoundguided "dynamic needle tip positioning" in pediatric radial arterial catheterization in standardized training of anesthesiology. Methods Twenty resident with standardized anesthesia training were randomly divided into ultrasound-guided teaching group(group A) and traditional teaching group(group B)(n=10 in each group). Group A was teached with ultrasound-guided radial artery catheterization, and group B was teached with conventional blind-catheterization. Results The success rate of the first attempting in group A was 30%, and the success rate of the first attempting in group B was 12.0%(P=0.027). The total success rate of group A was 44.0%, the total success rate of group B was 24.0%(P=0.035). 100.0% of the residents in the group A were interested in learning ultrasound-guided catheterization and ultrasoundguided catheterization were considered to be essential skills for anesthesiologists. Conclusion Ultrasound-guided "dynamic needle tip positioning" in teaching pediatric radial artery catheterization is beneficial to improve the success rate of beginners, enhance their confidence and learning interest.
