全麻和腰硬联合麻醉用于髋关节置换术的疗效比较

Comparison of Effect of General Anesthesia and Lumbar Epidural Anesthesia for Hip Replacement

目的观察比较全麻和腰硬联合麻醉在髋关节置换术上的效果及不良反应。方法选取2018年4月至2019年5月贵州省赤水市人民医院进行髋关节置换术的患者90例,将患者按随机数表法分为观察组和对照组,各45例。比较两组患者术中、术后并发症发生率。结果观察组MAP和HR波动明显低于对照组(P<0.05),术后入ICU、深静脉血栓发生率和肺部并发症观察组明显少于对照组(P<0.05),观察组术后VAS评分和认知功能障碍的发生明显优于对照组(P<0.05)。结论腰硬联合麻醉在髋关节置换术上的应用,对患者手术安全性提供了较大帮助,同时减少髋关节置换围手术期并发症的发生。

Objective To observe and compare the effects and adverse reactions of general anesthesia and lumbar epidural anesthesia on hip replacement.Methods From April 2018 to May 2019,90 patients with hip replacement were selected from Chishui People's Hospital of Guizhou Province.The patients were divided into observation group and control group with 45 cases each.The incidence of intraoperative and postoperative complications was compared between the two groups.Results The fluctuation of map and HR in the observation group was significantly lower than that in the control group(P<0.05),the incidence of deep vein thrombosis and pulmonary complications in the observation group was significantly lower than that in the control group(P<0.05),and the VAS score and cognitive dysfunction in the observation group were significantly higher than those in the control group(P<0.05).Conclusion The application of combined spinal and epidural anesthesia in hip arthroplasty is helpful to the safety of patients,and it can also reduce the incidence of perioperative complications of hip arthroplasty.
