低剂量造影剂64排螺旋CT肺动脉成像在肺栓塞诊断中的应用 被引量:8

摘要 目的:探究低剂量造影剂64排螺旋CT肺动脉成像在肺栓塞诊断中的应用,为肺栓塞诊断提供参考。方法:选择2016年3月-2019年3月在阳江市人民医院实行64排螺旋CT肺动脉成像进行诊治的疑似肺栓塞患者90例为研究对象。按照随机数字表法将患者分为对照组和观察组,每组45例。对照组实施常规剂量(50 mL造影剂+30 mL 0.9%氯化钠溶液),观察组实施低剂量(30 mL造影剂+30 mL 0.9%氯化钠溶液)。比较分析两组患者的信噪比、对比噪声比、图像噪声及主观图像质量评分、肺血管强化CT值以及不良反应发生率。结果:两组的信噪比、对比噪声比、图像噪声、主观图像质量评分比较,差异均无统计学意义(P>0.05);两组的右肺动脉、左肺动脉强化CT值比较,差异均无统计学意义(P>0.05);观察组不良反应发生率为13.33%,明显低于对照组的42.22%,差异有统计学意义(P<0.05)。结论:低剂量造影剂64排螺旋CT肺动脉成像在诊断肺栓塞中同常规剂量造影剂在信噪比、对比噪声比、图像噪声及主观图像质量评分、肺血管强化CT值上并无明显差异,且不良反应发生率较低,值得推广应用。 Objective:To investigate the application of low-dose contrast agent 64-slice spiral CT pulmonary artery imaging in the diagnosis of pulmonary embolism,and to provide reference for the diagnosis of pulmonary embolism.Method:A total of 90 patients with suspected pulmonary embolism who underwent 64-slice spiral CT pulmonary artery imaging from March 2016 to March 2019 in the People’s Hospital of Yangjiang City were selected as subjects.According to the random number table method,patients were randomly divided into control group and observation group,45 cases in each group.The control group was given a conventional dose(50 mL contrast agent+30 mL 0.9%Sodium Chloride Solution),and the observation group was administered a low dose(30 mL contrast agent+30 mL 0.9%Sodium Chloride Solution).The signal-to-noise ratio,contrast-to-noise ratio,image noise and subjective image quality score,pulmonary vascular enhanced CT value,and incidence of adverse reactions were compared between the two groups.Result:The signal-to-noise ratio,contrast noise ratio,image noise and subjective image quality score of the two groups showed no significant differences(P>0.05);enhanced CT values of right pulmonary artery and left pulmonary artery in the two groups showed no significant differences(P>0.05);the incidence of adverse reactions in the observation group was 13.33%,significantly lower than 42.22%in the control group,the difference was statistically significant(P<0.05).Conclusion:Low-dose contrast agent 64-slice spiral CT pulmonary artery imaging in the diagnosis of pulmonary embolism has no significant difference in signal-to-noise ratio,contrast-to-noise ratio,image noise and subjective image quality score,and pulmonary vascular enhancement CT value,the incidence is low and it is worth promoting.
