大柴胡汤颗粒治疗风火上扰型缺血性中风的效果

Effect of Dachaihu Decoction Granules in Treating Ischemic Stroke with Syndrome of Upward Disturbance of Wind-fire

目的:探讨大柴胡汤颗粒治疗风火上扰型缺血性中风的效果。方法:收集2017年5月-2019年4月本院收治的64例风火上扰型缺血性中风患者,按照随机数字表法分为两组,各32例。对照组给予内科常规治疗,观察组在对照组基础上加用大柴胡汤颗粒治疗,疗程均为4周。治疗前后,采用美国国立卫生研究院卒中量表(NIHSS)评估神经功能,采用Barthel指数评定量表评估日常生活能力,检测血清同型半胱氨酸(Hcy)、肿瘤坏死因子(TNF-α)含量,并比较疗效。结果:观察组总有效率为93.75%,明显高于对照组的75.00%(P<0.05);治疗后,观察组NIHSS评分明显低于对照组,Barthel指数评分明显高于对照组(P<0.05);治疗后,观察组血清Hcy、TNF-α均明显低于对照组(P<0.05)。结论:大柴胡汤颗粒能够明显改善风火上扰型缺血性中风患者神经功能,提高日常生活能力,其机制可能与有效调节血清中Hcy、TNF-α表达,从而减轻机体炎症反应,纠正Hcy异常有关。

Objective:To investigate the effect of Dachaihu Decoction Granules in the treatment of ischemic stroke with syndrome of upward disturbance of wind-fire.Method:Sixty-four patients with ischemic stroke with syndrome of upward disturbance of wind-fire who admitted to our hospital from May 2017 to April 2019 were enrolled.The patients were divided into two groups according to the random number table method,32 cases in each group.The control group was given routine medical treatment,and on the basis of the control group the observation group was treated with Dachaihu Decoction Granules,the treatment courses of two groups were 4 weeks.Before and after treatment,the National Institutes of Health Stroke Scale(NIHSS)was used to assess neurological function,the Barthel index assessment scale was used to assess daily living ability,and serum homocysteine(Hcy)and tumor necrosis factor(TNF-α)levels were measured,the effects of two groups were compared.Result:The total effective rate of the observation group was 93.75%,which was higher than 75.00%of the control group(P<0.05).After treatment,the NIHSS score of the observation group was lower than that of the control group,and the Barthel index score was higher than that of the control group(P<0.05).After treatment,the serum levels of Hcy and TNF-αin the observation group were lower than those in the control group(P<0.05).Conclusion:Dachaihu Decoction Granules can significantly improve the neurological function and the daily living ability of patients with ischemic stroke with syndrome of upward disturbance of wind-fire.The mechanism may be related to effectively regulating the expression of Hcy and TNF-αin the serum,thereby reducing the body’s inflammatory response and correcting Hcy abnormalities.
