补肾活血汤联合前外侧手术入路治疗对复杂Pilon骨折患者炎症反应发生的影响

Effect of Bushen Huoxue Decoction Combined with Anterolateral Surgical Approach on Inflammatory Reaction in Patients with Complex Pilon Fracture

目的:探讨补肾活血汤联合前外侧手术入路治疗对复杂Pilon骨折患者炎症反应发生的影响。方法:选取2017年1月-2019年1月本院收治的复杂Pilon骨折患者90例,按照随机数字表法将其分为两组,每组各45例。对照组应用前外侧手术入路治疗,研究组在对照组基础上联合应用补肾活血汤治疗,分析两组术后骨折愈合、踝关节功能恢复、机体免疫恢复及炎症反应情况。结果:研究组X线骨痂评分高于对照组(P<0.05);研究组踝关节功能恢复优良率高于对照组(P<0.05);研究组免疫指标CD3+、CD4+均高于对照组(P<0.05),研究组CD8+低于对照组(P<0.05);研究组炎性因子TNF-α、CRP及IL-6水平均低于对照组(P<0.05)。结论:采用补肾活血汤联合前外侧手术入路治疗复杂Pilon骨折可促进骨折愈合,降低炎症反应强度,促进免疫功能和踝关节功能恢复,可以在临床上进一步推广应用。

Objective:To investigate the effect of Bushen Huoxue Decoction combined with anterolateral surgical approach on inflammatory reaction in patients with complex Pilon fracture.Method:Ninety patients with complex Pilon fracture admitted to our hospital from January 2017 to January 2019 were selected and randomly divided into two groups according to the random number table,45 cases in each group.The control group was treated by anterolateral surgical approach,the study group was treated by Bushen Huoxue Decoction on the basis of the control group.The fracture healing,ankle joint function recovery,body immune recovery and inflammatory reaction in two groups were analyzed.Result:The X-ray callus score of the study group was higher than that of the control group(P<0.05).The ankle function recovery rate of the study group was higher than that of the control group(P<0.05).The immune indexes CD3+,CD4+of the study group were higher than those of the control group(P<0.05),and CD8+of the study group was lower than that of the control group(P<0.05).The levels of inflammatory factors TNF-α,CRP and IL-6 of the study group were lower than those of the control group(P<0.05).Conclusion:Bushen Huoxue Decoction combined with anterolateral surgical approach for complex Pilon fracture can promote fracture healing,reduce the intensity of inflammatory reaction,promote the recovery of immune function and the recovery of ankle function,which can be further promoted in clinical application.
