山莨菪碱联合阿奇霉素对小儿支原体肺炎的疗效及免疫球蛋白等的影响 被引量:21

摘要 目的探讨山莨菪碱联合阿奇霉素对小儿支原体肺炎(MPP)的临床疗效及免疫球蛋白和血清细胞因子水平的影响。方法选取MPP患儿95例,随机分为研究组48例,对照组47例,对照组给予阿奇霉素静脉滴注,研究组在对照组治疗的基础上给予山莨菪碱。比较2组患儿的临床症状改善情况、血浆免疫球蛋白和血清细胞因子的变化情况以及不良反应发生情况。结果研究组患儿的疗效显著高于对照组(P<0.05);研究组患儿的临床症状改善时间及住院时间均显著短于对照组(P<0.05)。治疗后2组患儿的血浆免疫球蛋白水平均较治疗前显著升高(P<0.05),且研究组血浆免疫球蛋白指标显著高于对照组(P<0.05)。2组患儿血清降钙素原(PCT)、白细胞介素-6(IL-6)及肿瘤坏死因子-α(TNF-α)水平均较治疗前显著降低(P<0.05),且研究组显著低于对照组(P<0.05)。结论山莨菪碱联合阿奇霉素治疗小儿MPP临床疗效显著,能有效促进患儿机体免疫功能的提高与炎症状态的改善,能有效减少阿奇霉素引起的相关不良反应。 Objective To investigate the clinical effect of anisodamine combined with azithromycin on mycoplasma pneumonia(MPP)in children and its influence on the levels of immunoglobulin and serum cytokines.Methods 95 children with MPP were randomly divided into observation group(48 cases)and control group(47 cases).The control group were given azithromycin intravenous drip,and the observation group was given anisodamine on the basis of control group.The improvement of clinical symptoms,changes of plasma immunoglobulin,changes of serum cytokines and adverse reactions were compared between the 2 groups.Results The curative effect of the study group was significantly higher than control group(P<0.05).The improvement time of clinical symptoms and time of stay in the study group were significantly short than those in control group(P<0.05).After treatment,the levels of plasma immunoglobulin in both groups were significantly higher than those before treatment(P<0.05),and the indexes of plasma immunoglobulin in the study group were significantly higher than those in control group(P<0.05).After treatment,the levels of plasma immunoglobulin in both groups were significantly higher than those before treatment(P<0.05),and the indexes of plasma immunoglobulin in the study group were significantly higher than those in the control group(P<0.05).The serum procalcitonin(PCT),interleukin-6(IL-6)and tumor necrosis factor(TNF-α)of the 2 groups were significantly lower than those of control group(P<0.05).Conclusion Anisodamine combined with azithromycin in the treatment of MPP in children has a significant clinical effect.It can effectively promote the improvement of immune function and inflammatory state in children,and effectively reduce the adverse drug reactions caused by azithromycin.
