摘要
目的明确目前国内首剂麻疹-风疹联合减毒活疫苗(MR)的麻疹和风疹成分的免疫原性,并为将来是否需要调整首剂接种年龄提供科学依据。方法分层随机抽取濮阳市两个县区8月龄(240~254日龄)婴儿及母亲206组,婴儿于接种前采集血标本2 mL,接种1个月(间隔30~35 d)采集第二份血标本2 mL,母亲血标本5 mL与其婴儿第一份血标本同时采集。用酶联免疫吸附试验(ELISA)定量检测麻疹、风疹IgG抗体水平;用SPSS16.0软件处理数据。结果206名婴儿接种前阴性率97.5%,接种后免前阴性率+4倍增高者94.7%,接种后风疹抗体阳转率61.7%;母亲阳性率96.1%。抗体几何平均浓度(GMC,95%CI):婴儿接种前麻疹、风疹分别为6.6(5.4~8.1)mIU/mL和1.1(0.9~1.3)mIU/mL;接种后麻疹、风疹分别为764.1(677.9~861.2)mIU/mL和21.7(18.5~25.4)mIU/mL;母亲麻疹、风疹分别为491.4(433.8~556.7)mIU/mL和45.4(39.0~52.8)mIU/mL。结论本研究结果支持8月龄婴儿接种MR的免疫策略,但风疹ELISA检测试剂对此类免疫原性研究的适用性及Cut-off值是否应该调整等有待进一步研究确认。
Objective To clarify the immunogenicity of measles and rubella components of the first dose of Measles-Rubella(MR)combined with attenuated live vaccine In the present domestic,and to provide scientific basis for the future need to adjust the first vaccination age.Methods Randomly selected 206 groups of eight-months-old(240-254 days)infants and their mothers in the two counties of Puyang city according to the principle of stratified randomization,collected 2 mL blood samples from infants before vaccination,and collected the second 2ml blood samples from the infants after vaccination one month,the mother's 5 mL blood sample was collected at the same time as the first blood sample of the infant.The antibody levels of measles and rubella IgG were measured by ELISA.The data was analyzed with SPSS 16.0 software.Results The negative rate of 206 infants was 97.5%before vaccination,and the negative rate of before vaccination increased fourfold after vaccination,and the proportion of rubella antibody negative to positive was 61.7%after vaccination.The mother's antibody positive rate was 96.1%.Antibody Geometric Mean Concentration(Geometric Mean Concentration,95%Confidence Interval)(GMC,95%CI):the measles and rubella were respectively 6.6(5.4-8.1)mIU/mL and 1.1(0.9-1.3)mIU/mL before vaccination,the measles and rubella were respectively 764.1(677.9-861.2)mIU/mL and 21.7(18.5-25.4)mIU/mL after vaccination,mother's measles and rubella were respectively 491.4(433.8-556.7)mIU/mL and 45.4(39.0-52.8)mIU/mL.Conclusions The results of this study supported the immunization strategy of eight-month-old infants vaccinated with MR.However,the applicability of rubella ELISA detection reagent to such immunogenicity research and whether the cut-off value should be adjusted should be further studied and confirmed.
