应用透明导管鞘治疗高血压性脑出血的临床分析 被引量:1

摘要 目的探讨应用传统开颅、微创钻孔血肿引流术和透明导管鞘辅助下显微镜技术清除脑出血血肿的临床疗效.方法将85例高血压性脑出血患者随机分为3组,分别为传统开颅组(对照组27例)、微创钻孔组(对照组34例)和透明导管鞘辅助下显微镜治疗组(实验组24例),对比分析3组患者的血肿清除率、住院时间、意识清醒时间、手术时间、术后感染率等.结果透明导管鞘辅助下显微镜治疗高血压性脑出血在患者预后、住院时间、术后感染率方面明显优于其他两种手术方式,患者住院时间短,住院总费用降低,感染率降低.结论透明导管鞘辅助下显微镜治疗高血压脑出血疗效确切,可彻底清除血肿,其微创、精准、高效、预后良好的优点使该技术在临床中得到广泛应用. Objective To compare the clinical efficacy of traditional craniotomy,minimally invasive drilling and clear catheter sheath assisted by microscope in the treatment of cerebral hemorrhage.Method 85 patients with hypertensive intracerebral hemorrhage were randomly divided into three groups: the traditional craniotomy group(27 cases in the control group),the minimally invasive drilling group(34 cases in the control group),and the transparent catheter sheath auxiliary microscope treatment group(24 cases in the experimental group). The hematoma clearance rate,length of hospital stay,time of consciousness,operative time and postoperative infection rate in three groups were compared and analyzed.Results Compared with traditional craniotomy,the operation of removing intracranial hematoma by intracranial transparent catheter sheath has the advantages of minimally invasive,convenient operation,less damage and shorter operation time. Long-term indwelling of intracranial drainage tube and injection of urokinase are not needed to reduce the risk of intracranial infection.Patients were hospitalized for a short time,and the total cost of hospitalization was reduced.Conclusion The technique of clear catheter sheath is suitable for the operation of hypertensive cerebral hemorrhage.
