SWI对轻型颅脑损伤的诊断价值 被引量:2

摘要 目的:探讨SWI在轻型颅脑损伤(mTBI)诊断中的应用价值。方法:收集42例mTBI患者的常规MRI、DWI、SWI图像及临床资料,比较SWI与常规MRI、DWI对异常信号及微出血灶的检出能力,评价SWI在mTBI诊断中的作用。结果:SWI共检出81个微出血灶,病灶检出率为72.3%,明显高于常规MRI及DWI,差异均有统计学意义(均P<0.001)。SWI序列上,格拉斯哥昏迷量表(GCS)评分为13、14、15分患者的微出血灶平均数目分别为2.4、1.8、1.5个,微出血灶主要分布于皮层/皮层下白质及深部白质,而胼胝体、基底节区及脑干/小脑则较少见。结论:SWI较常规MRI和DWI对mTBI患者微出血灶具有更高的诊断敏感性。 Objective:To explore the application value of SWI in mild traumatic brain injury(mTBI).Methods:Conventional MRI,DWI,SWI and clinical data of 42 patients with mTBI were collected.The detection ability of SWI,conventional MRI and DWI for abnormal signals and microhemorrhage lesions were compared,and the role of SWI in the diagnosis of mTBI was evaluated.Results:81 microhemorrhage lesions were detected on SWI,the detection rate was 72.3%,which was significantly higher than that on conventional MRI and DWI(all P<0.001).On SWI,the mean number of microhemorrhage lesions in GCS was 2.4,1.8 and 1.5 in patients of GCS 13,14 and 15 scores,respectively.Microhemorrhage lesions were mainly distributed in the cortex/subcortical white matter and deep white matter,but rarely in the corpus callosum,basal ganglia and brain stem/cerebellum.Conclusion:SWI is more sensitive than conventional MRI,DWI in the diagnosis of microhemorrhages in patients with mTBI.
