通窍活血汤对缺血性脑血管病患者神经功能恢复、血液流变学的影响研究

Effect of Tongqaio Huoxue Decoction on Nerve Function Recovery and Hemorheology for Patients with Ischemic Cerebrovascular Disease

目的:观察通窍活血汤对缺血性脑血管病(ICVD)患者神经功能恢复、血液流变学的影响。方法:选取2017年2月~2019年5月本院收治的ICVD患者190例,随机数表法将患者分为观察组和对照组,每组各95例。对照组给予常规治疗,观察组给予常规治疗+通窍活血汤,连续治疗一个月后分别对两组患者的临床疗效、神经功能恢复情况、糖脂代谢及血液流变学的相关指标进行评价。结果接受治疗后,观察组的总有效率为91.58%,高于对照组的78.95%(P<0.05);观察组的美国国立卫生研究院卒中量表(NIHSS)评分低于对照组(P<0.05);观察组的Barthel(BI)指数高于对照组(P<0.05);治疗后,两组患者的血液流变学指标均有所下降,且观察组的血浆黏度(PV)、全血粘度、红细胞聚集指数(AI)及血小板聚集率(PAR)水平均低于对照组(P<0.05);治疗后观察组的血清甘油三酯(TG)与糖化血红蛋白(HbA1c)值低于对照组(P<0.05),低密度脂蛋白胆固醇(LDL-C)值高于对照组,差异有统计学意义(P<0.05)。结论:通窍活血汤能提高治疗有效率与患者神经功能恢复情况,并能改善ICVD患者的临床症状、血液流变学指标与糖脂代谢水平。

Objective:To explore effect of Tongqaio Huoxue Decoction on nerve function recovery and hemorheology for patients with ischemic cerebrovascular disease(ICVD).Methods:190 ICVD patients who were admitted to the hospital from February 2017 to May 2019 were enrolled.They were divided into observation group and controlled group by random number table method,95 cases in each group.The controlled group was given routine treatment,while the observation group was given routine treatment and Tongqiao Huoxue Decoction.After 1-month continuous treatment,the clinical curative effect,nerve function recovery,glycolipid metabolism and hemorheology related indexes in both groups were evaluated.Results:After treatment,the total response rate of the observation group was higher than that of the controlled group(91.58% vs 78.95%)(P < 0.05).The scores of National Institutes of Health Stroke Scale(NIHSS) in the observation group was lower than that in the controlled group(P < 0.05).Barthel index(BI) of the observation group was higher than that of the controlled group(P < 0.05).After treatment,the hemorheology indexes of both groups were decreased.The levels of plasma viscosity(PV),whole blood viscosity,erythrocyte aggregation index(AI) and platelet aggregation rate(PAR) in the observation group were lower than those in the controlled group(P < 0.05).After treatment,serum triglyceride(TG) and glycosylated hemoglobin(HbA1 c) in the observation group were lower than those in the controlled group(P < 0.05),while low-density lipoprotein cholesterol(LDL-C) was higher than that in the controlled group(P < 0.05).Conclusion:Tongqiao Huoxue Decoction can improve response rate of treatment and nerve function recovery,improve clinical symptoms,hemorheology indexes and glucose-lipid metabolism level of ICVD patients.
