温经活血止痛方熏蒸联合热敏灸治疗肩周炎的效果及对肩关节功能的影响

Curative Effect of Using Wenjing Huoxue Zhitong Recipe for Fumigation Combined with Thermal Moxibustion in the Treatment of Scapulohumeral Periarthritis and Its Influences on Shoulder Joint Function and Range of Motion

目的:观察温经活血止痛方熏蒸联合热敏灸治疗肩周炎的疗效。方法:选取2018年3月~2019年4月来医院就诊的117例肩周炎患者为研究对象,采用随机数字表法将其分为对照组(热敏灸治疗)58例和观察组(温经活血止痛方熏蒸联合热敏灸治疗)59例。比较两组患者的中医证候疗效、视觉模拟疼痛评分(VAS)与疼痛强度(PPI)、肩关节功能、肩关节活动度以及血清中肿瘤坏死因子-α(TNF-α)、白细胞介素-10(IL-10)、白细胞介素-6(IL-6)水平的变化。结果:治疗后,观察组患者中医证候总有效率86.44%(51/59)明显高于对照组患者总有效率70.69%(41/58)(P<0.05);治疗后,观察组患者的VAS评分、PPI评分以及肩功能评分均比对照组患者明显降低(P<0.05);治疗后,观察组患者的关节外展、内旋、外旋、前屈、后伸活动度均比对照组患者明显增大(P<0.05);治疗后,联合组患者IL-6、IL-10和TNF-α比对照组明显降低(P<0.05)。结论:温经活血止痛方熏蒸联合热敏灸治疗肩周炎有很好的疗效,可以有效缓解患者的疼痛,提高肩关节的活动度以及功能。

Objective:To explore the curative effect of Wenjing Huoxue Zhitong Recipe for fumigation combined with thermal moxibustion on scapulohumeral periarthritis.Methods:117 patients with scapulohumeral periarthritis who were treated in the hospital from March 2018 to April 2019 were enrolled as study objects.They were divided into controlled group(thermal moxibustion therapy,58 cases) and observation group(Wenjing Huoxue Zhitong Recipe for fumigation combined with thermal moxibustion,59 cases).The changes in curative effect of traditional Chinese medicine(TCM) syndromes,score of visual analogue scale(VAS),present pain intensity(PPI),shoulder joint function,range of motion(ROM) of shoulder joint,levels of serum tumor necrosis factor-α(TNF-α),interleukin-10(IL-10) and interleukin-6(IL-6) were compared between the two groups.Results:After treatment,the total response rate of TCM syndromes in the observation group was significantly higher than that in the controlled group [86.44%(51/59) vs(70.69%(41/58)].After treatment,scores of VA,PPI and shoulder function in the observation group were significantly lower than those in the controlled group(P < 0.05).After treatment,ROM of shoulder joint such as abduction,internal rotation,external rotation,anteflexion and rear protraction in the observation group were higher than those in the controlled group(P < 0.05).After treatment,IL-6,IL-10 and TNF-α in the observation group were significantly lower than those in the controlled group(P < 0.05).Conclusion:The curative effect of Wenjing Huoxue Zhitong Recipe for fumigation combined with thermal moxibustion is good on scapulohumeral periarthritis,which can effectively relieve pain of patients,improve ROM and function of shoulder joint.
