针灸联合康复训练对中风后偏瘫患者下肢运动功能及生活质量的影响

Effect of Acupuncture and Moxibustion Combined with Rehabilitation on Lower Limb Motor Function and Quality of Life for Patients with Post-Stroke Hemiplegia

目的:观察针灸联合康复训练对中风后偏瘫患者下肢运动功能及生活质量的影响。方法:选择我院收治的86例中风后偏瘫患者,随机分为对照组与观察组,对照组43例患者采用常规康复训练治疗,观察组43例患者采用针灸联合康复训练治疗,观察两组患者临床疗效、中医症状评分、下肢运动功能及生活质量差异。结果:经治疗,观察组与对照组治疗总有效率分别为76.74%和93.02%,差异有统计学意义(P<0.05);治疗后,两组中医症状(口眼歪斜、头晕目眩、偏身麻木、疼痛冷热感觉障碍)评分均显著降低(P<0.05),且观察组治疗后的各项症状评分均显著低于对照组(P<0.05);治疗后,两组下肢Fugl-Meyer评分、Holden步行功能分级(FAC)评分及日常生活活动能力(ADL)评分均显著升高(P<0.05),且观察组治疗后的Fugl-Meyer评分、FAC评分及ADL评分均显著高于对照组(P<0.05);治疗后,两组中风专用生活质量量表(SS-QOL)评分呈上升趋势,组内差异显著(P<0.05);且观察组治疗4周后及随访至12周时的SS-QOL评分均显著高于对照组(P<0.05)。结论:针灸联合康复训练可显著改善患者下肢运动功能及生活质量,值得临床推广。

Objective:To observe the effect of acupuncture and moxibustion combined with rehabilitation training on lower limb motor function and quality of life for patients with post-stroke hemiplegia.Methods:86 patients with post-stroke hemiplegia in our hospital were selected and randomly divided into controlled group and observation group.43 cases in the controlled group were treated with routine rehabilitation training,and 43 cases in the observation group were given acupuncture and moxibustion combined with rehabilitation training.Clinical efficacy,TCM symptoms scores,lower limb motor function and quality of life were observed in the two groups.Results:After treatment,the total effective rate of treatment in the observation group and the controlled group were 76.74% and 93.02% respectively(P < 0.05).After treatment,the scores of TCM symptoms(facial paralysis,dizziness,hemianesthesia,disorders of pain and cold and heat sensation) in the two groups were significantly decreased(P < 0.05),and the scores of symptoms in the observation group after treatment were significantly lower than those in the controlled group(P < 0.05).After treatment,the lower limb Fugl-Meyer score,Holden walking function grading(FAC) score and activity of daily living ability(ADL) score in the two groups were significantly increased(P < 0.05),and the Fugl-Meyer score,FAC score and ADL score in the observation group after treatment were significantly higher than those in the controlled group(P < 0.05).After treatment,the scores of stroke-specific quality of life(SS-QOL) in the two groups showed an upward trend(P < 0.05).The SS-QOL scores after 4 weeks of treatment and at 12 weeks of follow-up in the observation group were significantly higher than those in the controlled group(P < 0.05).Conclusion:Acupuncture and moxibustion combined with rehabilitation training can significantly improve the lower limb motor function and quality of life,and it is worthy of clinical promotion.
