肿瘤标志物升高与胰十二指肠切除术后恶性肿瘤短期复发的关系

Relationship Between Elevated Tumor Markers and Short-term Recurrence of Malignant Tumor After Pancreatoduodenectomy

目的探讨肿瘤标志物升高与胰十二指肠切除术(PD)后恶性肿瘤短期复发的关系。方法选取2013年1月至2016年7月在郑州大学第一附属医院接受PD手术的167例恶性肿瘤患者。随访并记录术后1 a复发情况及肿瘤标志物[糖类抗原199(CA199)、糖类抗原125(CA125)、癌胚抗原(CEA)及甲胎蛋白(AFP)]检测结果,分析肿瘤标志物升高与患者术后恶性肿瘤短期复发的关系。结果术前CA199、CEA升高均与术后1 a内肿瘤复发有关(均P<0.05);术前CA125、AFP升高均与术后肿瘤短期复发无关(均P>0.05)。术前CA199、CEA升高均是术后肿瘤短期复发的危险因素(均P<0.05);术后即服化疗药物可能是术后肿瘤短期复发的保护因素(P<0.05)。结论术前CA199、CEA升高均是术后肿瘤短期复发的危险因素,术后即服化疗药物可延长患者的无瘤生存时间。

Objective To explore the relationship between elevated tumor markers and short-term recurrence of malignant tumor after pancreatoduodenectomy.Methods A total of 167 cases of malignant tumor undergoing PD surgery in the First Affiliated Hospital of Zhengzhou University from January 2013 to July 2016 were selected. The patients were followed up, and the tumor recurrence within one year after operation and results of tumor markers [glycoantigens 199(CA199), glycoantigens 125(CA125), carcinoembryonic antigen(CEA) and alpha-fetoprotein(AFP)] detection were recorded. The relationship between tumor marker elevation and short-term recurrence of malignant tumor after surgery was analyzed.Results The preoperative CA199 and CEA elevations were both related to the tumor recurrence within one year after operation(both P<0.05). The preoperative CA125 and AFP elevations were not related to the short-term recurrence of tumor(both P>0.05). The preoperative CA199 and CEA elevations were both the risk factors for short-term recurrence of tumor(both P<0.05). Taking chemotherapy immediately after operation may be a protective factor for short-term recurrence of tumor(P<0.05).Conclusion The preoperative CA199 and CEA elevations are both the risk factors for short-term recurrence of tumor. Taking chemotherapy immediately after operation could extend the survival time of patients without tumor.
