阿奇霉素联合红霉素对儿童肺炎支原体肺炎血清中炎性细胞因子的影响

Effects of azithromycin combined with erythromycin on inflammatory cytokines in serum of mycoplasma pneumoniae pneumonia in children

目的:探究阿奇霉素联合红霉素对肺炎支原体肺炎(MPP)患儿血清中TNF-α、IL-6、IL-8及IL-10浓度的影响。方法:选取某院收治的肺炎支原体肺炎患儿155例,随机分为阿奇霉素治疗组(58例)、红霉素治疗组(55例)及阿奇霉素联合红霉素治疗组(联合治疗组42例)。比较3组患儿治疗前后炎性细胞因子IL-6、IL-8、IL-10及TNF-α水平和免疫因子IgG、IgM及IgA的变化差异,评估3组患儿治疗有效率。结果:3组患儿治疗前后IL-6、IL-8、IL-10及TNF-α变化组间比较存在明显统计学差异,联合治疗组明显高于阿奇霉素及红霉素单药治疗组(P<0.05);联合治疗组治疗前后IgG、IgA及IgM水平变化均高于单药治疗组(P<0.05);联合治疗组治疗有效率优于单药治疗组,3组比较差异有统计学意义(P<0.05)。结论:阿奇霉素与红霉素联合用药可明显降低MPP患儿的炎性指标,加快其康复进程。

Objective:To investigate the effect of azithromycin combined with erythromycin on serum concentrations of TNF-α,IL-6,IL-8 and IL-10 in children infected with mycoplasma pneumoniae pneumonia(MPP).Methods:155 children with MPP were randomly divided into an azithromycin group(58 cases),an erythromycin group(55 cases)and an azithromycin combined with erythromycin group(42 cases).Changes of inflammatory cytokines(IL-6,IL-8,IL-10 and TNF-α)and immune globulin(IgG,IgM and IgA)before and after treatment were compared among the three groups.Results:There were significant differences in change of IL-6,IL-8,IL-10 and TNF-αbefore and after treatment between the 3 groups,with changes in azithromycin combined with erythromycin group being more significant than the other two groups(P<0.05).The levels of IgG,IgA and IgM in the combination treatment group before and after treatment were significantly higher than those in the monotherapy group(P<0.05).The effective rate of the combination treatment group was higher than that of the monotherapy group(P<0.05).Conclusion:The combination of azithromycin and erythromycin can significantly reduce the inflammatory index of MPP children and accelerate their recovery.
