基层医院腹腔镜直肠癌根治术后并发症影响因素分析

Analysis of influence factors of complications after laparoscopic radical rectal resection in grass-roots hospitals

目的:分析腹腔镜直肠癌根治术后并发症及其相关影响因素。方法:回顾分析2012年9月至2019年9月97例行腹腔镜直肠癌根治术患者的临床资料,分析其并发症发生情况及相关因素。结果:97例行腹腔镜直肠癌根治术,19例(19.6%)术后发生并发症。分析结果显示,患者性别、体质量指数、术前合并症、肿瘤位置、肿瘤大小及术者经验与腹腔镜直肠癌根治术后并发症的发生有关;Logistic回归分析结果显示,患者性别、术前合并症、肿瘤位置、术者经验是术后并发症发生的独立危险因素(P<0.05)。结论:腹腔镜直肠癌根治术的并发症与多种因素有关,应注意提高术者手术技巧,结合相关因素、患者情况制定相应的预防与干预措施。

Objective:To investigate postoperative complications of laparoscopic radical rectal resection and their influence factors.Methods:The clinical data of 97 patients with rectal cancer who underwent laparoscopic radical resection from Sep.2012 to Sep.2019 were analyzed retrospectively.The postoperative complications and their related factors were analyzed.Results:In the 97 patients with rectal cancer who underwent laparoscopic radical resection,postoperative complications occurred in 19 patients(19.6%).The results of analysis showed that the postoperative complications rate was associated with the gender,body mass index,preoperative comorbidities,location and diameter of tumor and surgeon experience(P<0.05).The results of logistic regression analysis revealed that the gender,preoperative comorbidities,location of tumor and surgeon experience were the independent risk factors for the postoperative complications(P<0.05).Conclusions:Complications of laparoscopic radical rectal resection are associated with multi-factors.The essential aspects to prevent and treat complications are improvement of the surgeon's surgical skills,and formulation of the corresponding prevention and intervention measures combined with the relevant factors and the conditions of patients.
