“互联网+”下自我管理为导向的5A护理模式对2型糖尿病患者生活质量的影响

The Influence of Self-management-oriented 5A Nursing Mode Under"Internet+"on the Quality of Life of Patients with Type 2 Diabetes Mellitus

目的:研究"互联网+"背景下自我管理为导向的5A护理模式对2型糖尿病(T2DM)患者生活质量的影响。方法:将南京大学医学院附属鼓楼医院2016年3月至2017年3月收治的80例T2DM患者随机分为观察组("互联网+"背景下自我管理为导向的5A护理模式,n=40)与对照组(常规护理,n=40),干预时间持续1年,观察两组干预效果。结果:观察组经干预后SAS、SDS量表得分均较干预前降低(P<0.05),观察组干预后两量表得分均低于对照组(P<0.05);观察组经干预后,糖尿病自我管理行为较干预前改善(P<0.05),且观察组经干预后糖尿病自我管理行为优于对照组(P<0.05);观察组经干预后,A-DQOL量表中生理功能、心理精神、社会关系、治疗及总得分均较治疗前提高(P<0.05),对照组经干预后生理功能、心理精神、治疗及总得分较干预前提高(P<0.05),观察组干预后心理精神、社会关系、治疗及总得分均高于对照组(P<0.05)。结论:"互联网+"背景下自我管理为导向的5A护理模式能有效改善T2DM患者不良情绪,规范其疾病自我管理行为,提高生活质量。

Objective:To study the effects of self-management-oriented 5 A nursing model on the quality of life of patients with type 2 diabetes mellitus(T2 DM)in the context of"Internet+".Methods:80 patients with T2 DM admitted to our hospital from March 2016 to March 2017 were randomly divided into observation group(self-management-oriented 5 A nursing model in the context of"Internet+",n=40)and control group(conventional nursing,n=40),and they were continuously intervened for 1 year.The intervention effects of the two groups were observed.Results:After intervention,the scores of Self-rating Anxiety Scale(SAS)and Self-rating Depression Scale(SDS)in observation group were significantly decreased compared with those before intervention(P<0.05),and the scores of the two scales in observation group after intervention were significantly lower than those in control group(P<0.05).After intervention,the self-management behaviors of diabetes in observation group were significantly improved compared with those before intervention(P<0.05),and the self-management behaviors of diabetes in observation group after intervention were better than those in control group(P<0.05).After intervention,the scores of physiological function,mental spirit,social relationship and treatment and total score of Adjusted Diabetes Quality of Life Scale(A-DQOL)were significantly increased compared with those before intervention(P<0.05),and the scores of physiological function,mental spirit and treatment and total score in control group after intervention were significantly increased compared with those before intervention(P<0.05),and the scores of mental spirit,social relationship and treatment and total score in observation group after intervention were significantly higher than those in control group(P<0.05).Conclusion:Self-management-oriented 5 A nursing model in the context of"Internet+"can effectively improve adverse emotions in T2 DM patients,regulate the self-management behaviors,and improve the quality of life.
