左、右侧结肠癌患者的临床症状、分化程度、病理特征及转移情况分析

Analysis of Clinical Symptoms,Differentiation,Pathological Features and Metastasis of Left and Right Colon Cancer Patients

目的:探究左右侧结肠癌患者的临床症状、分化程度、病理特征以及转移情况,为临床早期诊断及治疗提供参考。方法:以2015年7月~2018年4月为研究阶段,对44例左侧结肠癌、51例右侧结肠癌患者资料进行分析,对比患者的临床症状、分化程度、病理及转移情况。结果:(1)临床症状:右侧结肠癌首发症状多为腹痛和腹胀、左侧结肠癌首发症状为大便习惯和性状改变,对比存在差异(P<0.05);(2)分化程度:右侧结肠癌比例最高为低分化,其次为中分化、高分化,左侧结肠癌分化程度最高为中分化,其次为高分化、低分化(P<0.05);(3)病理特征:右侧结肠癌肿块直径在5cm以上比例较高,且以溃疡型为主(P<0.05);(4)转移:右侧结肠癌淋巴转移率较高,肝脏转移率高(P<0.05)。结论:左右侧结肠癌患者的临床症状、分化程度、病理特征和转移情况存在较大差异,且右侧结肠癌的恶性程度较高,临床诊断及治疗不可相同对待。

Objective:To explore the clinical symptoms,degree of differentiation,pathological features and metastasis of left and right colon cancer patients,so as to provide reference for early clinical diagnosis and treatment.Methods:From July 2015 to April 2018,44 patients with colon cancer on the left side and 51 patients with colon cancer on the right side were analyzed,to compare their clinical symptoms,degree of differentiation,pathology and metastasis.Results:(1)Clinical symptoms:the first symptoms of colon cancer on the right side were mostly abdominal pain and abdominal distension,while the first symptoms of colon cancer on the left side were changes in stool habits and traits(P<0.05);(2)Degree of differentiation:the highest proportion of colon cancer on the right side was low differentiation,followed by middle differentiation and high differentiation,and the highest degree of differentiation on the left side was middle differentiation,followed by high differentiation and low differentiation(P<0.05);(3)Pathological features:right colon cancer mass diameter of more than 5cm high proportion,and mainly ulcerative type(P<0.05);(4)Metastasis:the lymphatic metastasis rate of the right colon cancer was higher,and the liver metastasis rate was higher(P<0.05).Conclusion:The clinical symptoms,degree of differentiation,pathological features and metastasis of the patients with colon cancer on the left and right sides were significantly different,and the degree of malignancy of the right side of colon cancer was higher,so the clinical diagnosis and treatment could not be treated in the same way.
