中晚孕期胎儿心脏三尖瓣返流的超声诊断意义

The Significance of Ultrasonic Diagnosis of Fetal Tricuspid Regurgitation in the Middle and Late Period ofPregnancy

目的:探讨产前超声对中晚孕期胎儿三尖瓣返流的诊断价值。方法:对大连市妇幼保健院2015年9月~2018年1月产前超声诊断的113例中晚孕期胎儿三尖瓣返流的超声声像图特征及病因进行回顾性分析,并追踪其预后。结果:三尖瓣返流超声声像图特征:四腔心切面收缩期右心房内可探及源于三尖瓣瓣口的返流血流信号。113例中晚孕期胎儿三尖瓣返流按病因分为:非单纯性返流(心内、心外结构异常)组44例和单纯性返流组69例。非单纯性返流组中胎死宫内3例,引产12例,新生儿期死亡2例。单纯性返流组生后均正常。结论:产前超声诊断中晚孕期胎儿三尖瓣返流,并分析其病因,有助于评估胎儿预后,对优生优育具有重要的临床指导价值。

Objective:To explore the clinical value of prenatal ultrasound in the diagnosis of fetal tricuspid regurgitation in the middle and late pregnancy.Methods:The sonographic features and causes of fetal tricuspid regurgitation in 113 cases of mid late pregnancy diagnosed by prenatal ultrasound from September 2015 to January 2018 in dalian maternal and child health hospital were analyzed retrospectively,and the prognosis was followed up.Results:The ultrasonic characteristics of tricuspid regurgitation:In the right atrium of the four chamber section,the systolic flow signal from tricuspid valve can be seen.The 113 cases were divided into non-simple regurgitation(intracardiac and extracardiac structural abnormalities)group(44 cases)and simple regurgitation group(69 cases).In the non simple reflux,there were fetal death in 3 cases,induced labor in 12 cases,and neonatal death in 2 cases.And in the simple mild reflux,there were all normal after birth.Conclusions:Prenatal ultrasound diagnosis of fetal tricuspid regurgitation in the middle and late pregnancy,and analysis of its etiology,is helpful to evaluate the prognosis of the fetus,and has an important clinical value in guiding eugenics and eugenics.
