瘢痕子宫孕妇行阴道试产时的心理辅导对产程的影响

Effects of psychological counseling on labor process of pregnant women with scar uterus undergoing vaginal trial delivery

目的研究瘢痕子宫孕妇行阴道试产时的心理辅导对产程的影响。方法选取2017年1月~2018年1月我院的瘢痕子宫在妊娠选择阴道分娩的产妇116例,随机分为对照组和观察组,每组58例。对照组产妇采用常规分娩护理,观察组产妇在常规护理基础上增加心理护理及产程管理。比较两组产程时间、住院时间、出血量,并比较两组的分娩成功率及并发症发生率。结果观察组产程时间(7.92±1.25)h,住院时间(6.95±1.13)d,出血量(142.62±20.52)mL,均低于对照组,差异有统计学意义(P<0.05)。观察组阴道分娩成功率93.10%高于对照组,观察组产后并发症发生率3.45%低于对照组,差异有统计学意义(P<0.05)。结论瘢痕子宫产妇阴道分娩,可能具有一定的风险,加强心理护理和产程管理,能够改善产妇的分娩情况,提高阴道分娩成功率,减少产后并发症,临床效果理想。

Objective To study the effect of psychological counseling on labor process of pregnant women with scar uterus during vaginal trial delivery.Methods 116 parturient women with scarred uterus who underwent vaginal delivery in our hospital from January 2017 to January 2018 were selected.They were randomly divided into the control group and the observation group,with 58 cases in each group.Parturient women in the control group were given routine delivery nursing while parturient women in the observation group were given psychological nursing and labor process management on the basis of routine nursing.The duration of labor,hospitalization time and blood loss were compared between the two groups.The successful rate of delivery and the incidence of complications were compared between the two groups.Results The duration of labor(7.92±1.25)h,hospitalization time(6.95±1.13)d and blood loss(142.62±20.52)mL of the observation group were all lower than those of the control group.The difference was statistically significant(P<0.05).The successful rate of vaginal delivery of the observation group was 93.10%,higher than that of the control group.The incidence of postpartum complications in the observation group was 3.45%,lower than that of the control group.The difference was statistically significant(P<0.05).Conclusion Vaginal delivery for parturient women with scarred uterus may have certain risks.Strengthening psychological nursing and labor process management can improve the delivery situation of parturient women,improve the successful rate of vaginal delivery and reduce postpartum complications,which has ideal clinical curative effects.
