血清淀粉样蛋白A和超敏C反应蛋白联合检测在小儿呼吸道感染性疾病早期鉴别诊断中的临床应用价值及相关性分析 被引量:20

摘要 目的分析血清淀粉样蛋白A和超敏C反应蛋白联合检测在小儿呼吸道感染性疾病早期鉴别诊断中的临床应用价值。方法选取2018年6月~2019年5月我院收治的100例细菌感染急性期呼吸道小儿为细菌组。另外择取同期接受治疗的100例病毒感染急性期呼吸道感染者为病毒组。选取100例接受体检的健康小儿为对照组。开展血清淀粉样蛋白A和超敏C反应蛋白联合检测,分析结果。结果细菌组受试者血清中hs-CRP水平明显比对照组以及病毒组高,差异有统计学意义(P<0.05)。就SAA/hs-CRP方面来看,细菌组比对照组高,但比病毒组低,差异有统计学意义(P<0.05)。和对照组比较,病毒组SAA水平,以及SAA/hs-CRP显著更高,差异有统计学意义(P<0.05)。但上述两者的hs-CRP以及CRP,差异无统计学意义(P>0.05)。将病原体检查结果作为金标准。经过分析证实:SAA/hs-CRP在诊断早期小儿呼吸道病毒感染曲线下面积最高。并且值得说明的是,在阴性预测值、阳性预测值、特异度、灵敏度方面也明显高于SAA以及hs-CRP检测,差异有统计学意义(P<0.05)。结论在小儿呼吸感染性疾病早期诊断中开展SAA与hs-CRP联合检测,有助于提升诊断准确率。此法有着较高的临床应用价值,因此值得进一步在临床中推广应用。 Objective To analyze the clinical application value of combined detection of serum amyloid protein A and high-sensitivity C-reactive protein in early differential diagnosis of pediatric respiratory infection.Methods 100 pediatric patients with acute respiratory tract bacterial infection admitted in our hospital from June 2018 to May 2019 were selected as the bacterial group.100 pediatric patients with acute respiratory tract virus infection treated during the same period were selected as the virus group.100 healthy children who underwent physical examination were selected as the control group.The combined detection of serum amyloid A and high-sensitivity C-reactive protein was conducted to analyze the results.Results The serum levels of hs-CRP in the bacterial group were significantly higher than those in the control group and the virus group,and the difference was statistically significant(P<0.05).In terms of SAA/hs-CRP,the bacterial group was higher than the control group but lower than the virus group,and the differences were statistically significant(P<0.05).Compared with the control group,the SAA and SAA/hs-CRP levels were significantly higher in the virus group,and the difference was statistically significant(P<0.05).However,the difference between hs-CRP and CRP of the two groups was statistically significant(P>0.05).The pathogen test results were considered as the gold standard.The analysis confirmed that SAA/hs-CRP had the highest area under the curve for the diagnosis of early pediatric respiratory tract virus infection.It was also worth noting that the negative predictive value,positive predictive value,specificity and sensitivity of SAA/hs-CRP were significantly higher than the SAA and hs-CRP detections,and the differences were statistically significant(P<0.05).Conclusion The combined detection of SAA and hs-CRP in the early diagnosis of pediatric respiratory infection is helpful to improve the diagnostic accuracy.This approach has a high clinical application value and is worth further promotion and application in the clinical practices.
