急性有机磷农药中毒合并呼吸功能衰竭的急诊抢救分析及对患者肺功能的影响 被引量:7

摘要 目的探讨急性有机磷农药中毒合并呼吸功能衰竭的急诊抢救分析及对患者肺功能的影响。方法选取2017年5月~2019年5月我院收治的急性有机磷农药中毒合并呼吸功能衰竭患者64例,根据其抢救方案不同将其分为两组,对照组应用血流灌流进行抢救治疗,研究组应用连续性静脉-静脉血液滤过进行抢救治疗,分析两组患者抢救结果以及肺功能改善情况。结果研究组抢救成功率高于对照组,差异有统计学意义(P<0.05);研究组机械通气、ICU停留、神志清醒以及CHE活力恢复60%时间均低于对照组,差异有统计学意义(P<0.05);研究组肺功能指标FEV1、FVC、MMEF以及PEF均高于对照组,差异有统计学意义(P<0.05)。结论急性有机磷农药中毒合并呼吸功能衰竭患者抢救时采用连续性静脉-静脉血液滤过治疗可有效提高患者抢救成功率,促进患者肺功能恢复,缩短患者抢救后恢复时间,可以在临床中进行进一步推广应用。 Objective To investigate the analysis on emergency rescue of acute organophosphorus pesticide poisoning with respiratory failure and its effect on pulmonary function of patients.Methods 64 patients with acute organophosphorus pesticide poisoning with respiratory failure admitted and treated at our hospital from May 2017 to May 2019 were selected,and divided into two groups according to different rescue regimens.The control group was performed with rescue therapy by blood stream perfusion,while the study group was performed with rescue therapy by continuous veno-venous hemofiltration.The rescue results and improvement of pulmonary function were analyzed in the two groups of patients.Results The success rate of rescue in the study group was higher than that in the control group,with statistically significant difference(P<0.05);The mechanical ventilation,ICU stay,consciousness,and time spent in 60%of CHE vitality recovery in the study group were all lower than those in the control group,with statistically significant difference(P<0.05);The pulmonary function indexes such as FEV1,FVC,MMEF and PEF in the study group were all higher than those in the control group,with statistically significant difference(P<0.05).Conclusion For patients with acute organophosphorus pesticide poisoning with respiratory failure,the rescue therapy by continuous veno-venous hemofiltration can effectively improve the success rate of rescue,promote the recovery of pulmonary function,and shorten the recovery time after rescue of patients,which can be further popularized and applied in clinical practice.
