斯氏针辅助牵引床复位股骨近端防旋髓内钉固定对股骨转子间骨折患者术后髋关节功能的影响

Effect of Reduction of Proximal Femoral Non-rotating Intramedullary Nail Fixation with Steinman Pin Assisted Traction Bed on Hip Function after Intertrochanteric Fracture

目的:探讨斯氏针辅助牵引床复位股骨近端防旋髓内钉(PFNA)固定对股骨转子间骨折患者术后髋关节功能的影响。方法:选取笔者所在医院股骨转子间骨折患者92例,依照手术方案不同分为研究组(n=46)、常规组(n=46)。常规组采用牵引床复位PFNA固定,研究组采用斯氏针辅助牵引床复位PFNA固定,统计比较两组手术相关指标(术中出血量、手术时间、骨折愈合时间)、骨折复位优良率及术前及术后3、6个月髋关节功能评分(HSS)。结果:研究组术中出血量少于常规组,骨折愈合时间短于常规组(P<0.05);研究组术后3、6个月HSS评分高于常规组(P<0.05);研究组术后6个月骨折复位优良率93.33%,高于常规组的77.27%(P<0.05)。结论:斯氏针辅助牵引床复位PFNA固定对股骨转子间骨折患者复位效果良好,可减少术中出血量,缩短术后骨折愈合时间,促进髋关节功能恢复。

Objective:To investigate the effect of reduction of proximal femoral internal fixation(PFNA)by Steinman pin assisted traction bed on hip function after intertrochanteric fracture.Method:A total of 92 patients with intertrochanteric fracture in our hospital were selected and divided into study group(n=46)and routine group(n=46)according to different surgical procedures.Patients in the routine group were treated with traction bed reduction and PFNA fixation,while those in the study group were treated with Steinman pin assisted traction bed reduction and PFNA fixation.The indexes related to surgery(intraoperative blood loss,operation time,fracture healing time),fracture reduction rate and hip joint function score(HSS)were statistically compared between the two groups.Result:The intraoperative blood loss in the study group was less than that in the routine group,and the fracture healing time was shorter than that in the routine group(P<0.05).HSS scores of the study group were higher than those of the routine group at 3 and 6 months after surgery(P<0.05).The fracture reduction rate of 93.33%in the study group was higher than that of 77.27%in the routine group(P<0.05).Conclusion:Reduction of femoral intertrochanteric fractures with PFNA fixation assisted by Steinman pin can reduce intraoperative blood loss,shorten postoperative fracture healing time,and promote the recovery of hip function.
