锤凿劈冠法、高速涡轮钻微创法在下颌低位阻生智齿治疗中的效果对比 被引量:11

摘要 目的:对比锤凿劈冠法、高速涡轮钻微创法在下颌低位阻生智齿治疗中的应用效果。方法:回顾性分析笔者所在医院口腔门诊在2016年12月-2019年6月收治的102例下颌低位阻生智齿患者临床资料,按治疗方法不同将其分为两组,即锤凿组50例(锤凿劈冠法治疗)及微创组52例(高速涡轮钻微创法)。比较两组手术指标。结果:微创组拔牙时间(21.53±1.28)min,短于锤凿组的(32.48±2.59)min,差异有统计学意义(P<0.05)。微创组术中牙槽神经损伤、舌神经损伤、下颌角骨折、邻牙损伤发生率均低于锤凿组,差异有统计学意义(P<0.05)。微创组术后张口受限、颊面侧肿胀、颞颌关节痛及干槽症发生率低于锤凿组,差异有统计学意义(P<0.05)。微创组术后疼痛程度VAS评分(2.68±1.01)分,低于锤凿组(5.43±1.15)分,差异有统计学意义(P<0.05)。结论:与锤凿劈冠法相比,高速涡轮钻微创法治疗下颌低位阻生智齿获得明显效果,能够减轻手术创伤,安全性高,值得推广应用。 Objective:To compare the application effect of hammer chisel and split crown method and high-speed turbo drill minimally invasive method in the treatment of impacted lower mandibular wisdom teeth.Method:The clinical data of 102 patients with impacted lower mandibular wisdom teeth of our hospital from December 2016 to June 2019 were retrospectively analyzed.According to different treatment methods,they were divided into two groups,the hammer chisel group with 50 cases(treated by hammer chisel and split crown method)and the minimally invasive group with 52 cases(treated by high-speed turbo drill minimally invasive method).The surgical indicators of the two groups were compared.Result:The tooth extraction time in the minimally invasive group was(21.53±1.28)min,which was shorter than(32.48±2.59)min in the hammer chisel group,the difference was statistically significant(P<0.05).The incidence of alveolar nerve injury,lingual nerve injury,mandibular angle fracture and adjacent tooth injury in the minimally invasive group were lower than those in the hammer chisel group,the differences were statistically significant(P<0.05).The incidence of mouth opening limitation,buccal side swelling,temporomandibular joint pain and dry groove in the minimally invasive group were lower than those in the hammer chisel group,the differences were statistically significant(P<0.05).VAS score of postoperative pain degree in the minimally invasive group was(2.68±1.01)points,which was lower than(5.43±1.15)points in the hammer chisels group,the difference was statistically significant(P<0.05).Conclusion:Compared with hammer chisel and split crown method,high-speed turbo drill minimally invasive method for the treatment of impacted lower mandibular wisdom teeth has obvious effect,can reduce surgical trauma,and is highly safe,which is worthy of promotion and application.
