带锁髓内钉治疗胫骨与股骨干创伤骨折后骨不连的临床效果 被引量:7

Clinical Effect of Interlocking Intramedullary Nail in the Treatment of Nonunion after Traumatic Fracture of Tibia and Femoral Shaft

摘要 目的:探究带锁髓内钉治疗胫骨与股骨干创伤骨折后骨不连的临床效果。方法:随机选取2016年8月-2019年8月笔者所在医院胫骨与股骨干创伤骨折后骨不连患者60例,随机分为带锁髓内钉组(n=30)和动力加压钢板组(n=30)。比较两组手术相关指标、临床疗效及术后并发症发生率。结果:带锁髓内钉组手术时间、骨折愈合时间及切口长度均显著短于动力加压钢板组,术中出血量及术后引流量均显著少于动力加压钢板组(P<0.05)。带锁髓内钉组优良率为96.7%,显著高于动力加压钢板组的73.3%,差异有统计学意义(P<0.05)。带锁髓内钉组术后并发症发生率为6.7%(2/30),显著低于动力加压钢板组的33.3%(10/30),差异有统计学意义(P<0.05)。结论:带锁髓内钉治疗胫骨与股骨干创伤骨折后骨不连的临床效果较动力加压钢板好。 Objective:To explore the clinical effect of interlocking intramedullary nail in the treatment of nonunion after traumatic fracture of tibia and femoral shaft.Method:A total of 60 cases of patients with nonunion after traumatic fracture of tibia and femoral shaft in our hospital from August 2016 to August 2019 were selected and randomly divided into the interlocking intramedullary nail group(n=30)and the dynamic compression plate group(n=30).The surgically related indexes,clinical effect and incidence of postoperative complications of the two groups were compared.Result:The operation time,fracture healing time and incision length of the interlocking intramedullary nail group were significantly shorter than those of the dynamic compression plate group,and the amount of bleeding during operation and postoperative drainage volume were significantly less than those of the dynamic compression plate group(P<0.05).The excellent and good rate was 96.7% in the interlocking intramedullary nail group,which was significantly higher than 73.3%of the dynamic compression plate group,and the difference was statistically significant(P<0.05).The incidence of postoperative complications was 6.7%(2/30)in the interlocking intramedullary nail group,which was significantly lower than 33.3%(10/30)of the dynamic compression plate group,and the difference was statistically significant(P<0.05).Conclusion:The clinical effect of interlocking intramedullary nail on nonunion after traumatic fracture of tibia and femoral shaft is better than dynamic compression plate.
