内镜下黏膜切除术治疗胃肠道息肉的效果及安全性研究

The Effect and Safety of Endoscopic Mucosal Resection in the Treatment of Gastrointestinal Polyps

目的:研究胃肠道息肉治疗中应用内镜下黏膜切除术(EMR)的效果及安全性。方法:选取2018年3月-2019年4月在笔者所在医院行手术治疗的76例胃肠道息肉患者为研究对象。采用双模拟法将其分为两组,各38例。研究组行EMR,常规组行内镜下电切除术。比较两组手术效果及安全性。结果:研究组患者息肉完整切除率为97.37%,高于常规组的84.21%,差异有统计学意义(P<0.05)。研究组术中出血量少于常规组,差异有统计学意义(P<0.05)。研究组术后并发症发生率为5.26%,低于常规组的23.68%,差异有统计学意义(P<0.05)。研究组术后IL-8、IL-12、IL-6、IL-1β水平均低于常规组,差异有统计学意义(P<0.05)。结论:胃肠道息肉治疗中EMR有着良好的应用效果,息肉完整切除率高、炎症反应低、并发症少、安全性高,更有利于术后恢复,值得大范围推广应用。

Objective:To study the effect and safety of endoscopic mucosal resection(EMR)in the treatment of gastrointestinal polyps.Method:Seventysix patients with gastrointestinal polyps who underwent surgical treatment in our hospital from March 2018 to April 2019 were selected as the study subjects.They were divided into two groups by double simulation method,with 38 cases in each group.The study group underwent EMR,and the conventional group underwent endoscopic electric resection.The effect and safety of the two groups were compared.Result:The rate of polyp complete resection in the study group was 97.37%,which was higher than 84.21%in the conventional group,and the difference was statistically significant(P<0.05).The amount of intraoperative blood loss in the study group was less than that in the conventional group,and the difference was statistically significant(P<0.05).The incidence of postoperative complications in the study group was 5.26%,which was lower than 23.68%in the conventional group,the difference was statistically significant(P<0.05).The postoperative levels of IL-8,IL-12,IL-6 and IL-1β in the study group were lower than those in the conventional group,the differences were statistically significant(P<0.05).Conclusion:EMR in the treatment of gastrointestinal polyps has a good application effect,high polyp complete resection rate,low inflammatory response,less complications,high safety,and more conducive to postoperative recovery,which is worthy of extensive promotion and application.
