泰安市95例HIV/AIDS死亡相关因素的分析

Analysis of the factors of 95 HIV/AIDS deaths,Tai’an city

目的了解泰安市1996~2018年HIV/AIDS死亡病例的流行病学及临床特征、死亡相关因素,为降低艾滋病死亡率提供依据。方法从艾滋病综合防治信息系统下载HIV/AIDS历史卡片,分析死亡病例的人口学特征、感染途径、死亡原因、存活时间等。结果 1996~2018年泰安市累计共95例死亡病例,病死率为16.87%;死亡病例男性占82.11%、男女性别比4.59∶1、35~岁组43例(45.26%)、已婚63例(66.32%)、汉族92例(96.84%)、农民54例(56.84%)。其中不同年龄、婚姻、职业、感染途径死亡病例构成比差异有统计意义(P<0.01);主要死因分析显示,艾滋病相关死亡25例,其中pcp9例(9.47%),艾滋病无关死亡中无法判定占18例(18.95%);死亡病例存活年限<1年52例(54.74%);HIV/AIDS死亡病例接受抗病毒治疗构比差异有统计学意义(P<0.01)。结论泰安市自2014~2018年近几年死亡病例数呈现增高趋势,CD4+T淋巴细胞检测和抗病毒治疗不及时,存活年限低。

Objective To understand the epidemiological and clinical characteristics and mortality factors of HIV/AIDS deaths in Tai’an city from 1996 to 2018,so as to provide support to reduce AIDS mortality.Methods The HIV/AIDS history card was downloaded from the AIDS Integrated Prevention and Control Information System to analyze the demographic characteristics of the deaths,the route of infection,the cause of death,and the survival time,etc.Results Since 1996 to 2018,there had been a total of 95 deaths in Tai’an city,with a fatality rate of 16.87%.82.11%of the deaths were male.Among the death cases,the ratio of male to female was 4.59:1,there were 43 cases within age group 35-(45.26%),63 married(66.32%),92 of Han nationality(96.84%)and 54 farmers(56.84%).Among them,the difference in the composition ratio of deaths between different ages,marriage,occupation and infection route(P<0.01)were statistically significant.The analysis of main causes of death showed that there were 25 AIDS related deaths,9 of which were PCP(9.47%),18 of which could not be determined(18.95%).52 cases died with a life span less than 1 year(54.74%).The difference in the composition of HIV/AIDS deaths receiving antiviral treatment was statistically significant(P<0.01).Conclusion The number of deaths in Tai’an city from 2014 to 2018 has shown an increasing trend,with short life span.CD4-T lymphocyte detection and antiviral treatment are not timely.
