针对性护理开展于VSD负压治疗老年糖尿病慢性难愈合创面中的效果

The Effect of Targeted Nursing in the Treatment of Elderly Diabetes Chronic Refractory Wound with VSD Negative Pressure

目的:探讨针对性护理开展于负压封闭引流(VSD)技术治疗老年糖尿病慢性难愈合创面中的临床效果。方法:选取2017年1月-2019年5月笔者所在医院诊治的老年糖尿病慢性难愈合创面患者92例作为研究对象,均给予VSD技术治疗,按护理模式不同将其分为对照组(n=46)和研究组(n=46)。对照组采用常规护理模式,研究组采用针对性护理模式。比较两组患者负压治疗的疼痛感、护理不良事件发生率、护理前后抑郁情绪及主观护理体验优良率。结果:治疗后1周,两组患者VAS评分均低于治疗前,且研究组低于对照组,差异有统计学意义(t=15.726,P=0.000)。研究组护理不良事件发生率低于对照组,差异有统计学意义(t=7.180,P=0.007)。护理后,两组HAMD评分均低于护理前,且研究组低于对照组(t=11.862,P=0.000)。研究组患者主观护理体验优良率为100%,高于对照组的86.96%,差异有统计学意义(t=4.457,P=0.035)。结论:在VSD负压治疗老年糖尿病慢性难愈合创面中辅助以针对性护理干预,可进一步减轻疼痛感,消除消极情绪,降低护理不良事件,并予以患者理想护理体验,值得借鉴。

Objective:To explore the clinical effect of targeted nursing in the treatment of elderly diabetes chronic refractory wound with vacuum sealing drainage(VSD).Method:A total of 92 elderly diabetes chronic refractory wound patients diagnosed and treated in our hospital from January 2017 to May 2019 were selected as the study objects,they were all treated with VSD technology.They were divided into the control group(n=46)and the study group(n=46)according to different nursing modes.The control group adopted routine nursing mode,while the study group adopted targeted nursing mode.The pain,the incidence of nursing adverse events,the depression before and after nursing and the excellent and good rate of subjective nursing experience were compared between the two groups.Result:At 1 week after treatment,VAS scores in both groups were lower than before treatment,and that in the study group was lower than that in the control group,the difference was statistically significant(t=15.726,P=0.000).The incidence of nursing adverse events in the study group was lower than that in the control group,and the difference was statistically significant(t=7.180,P=0.007).After nursing,HAMD scores in both groups were lower than before nursing,and the study group was lower than the control group(t=11.862,P=0.000).The excellent and good rate of subjective nursing experience in the study group was 100%,which was higher than 86.96%in the control group,and the difference was statistically significant(t=4.457,P=0.035).Conclusion:In the treatment of elderly diabetes chronic refractory wound with VSD negative pressure,targeted nursing intervention can further reduce pain,eliminate negative emotions,reduce adverse nursing events,and provide patients with ideal nursing experience,which is worth learning.
