摘要
目的:探讨铁蛋白(SF)、25羟基维生素D(25-OH Vit D)及微小RNA-122(miR-122)水平在不同程度非酒精性脂肪肝(nonalcoholic fatty liver disease,NAFLD)患者中表达及临床意义,为NAFLD诊断及严重程度的判断提供科学依据。方法:选择2017年11月-2019年5月来医院就诊并确诊为NAFLD患者270例为NAFLD组,同期健康人群120例为HC组,分别检测血清中SF、25-OH Vit D及miR-122水平。结果:NAFLD组患者SF和miR-122水平明显高于HC组,差异有统计学意义(P<0.05),而25-OH Vit D水平明显低于HC组,差异有统计学意义(P<0.05);NAFLD男性患者SF和25-OH Vit D水平明显高于女性患者,差异有统计学意义(P<0.05),而miR-122水平差异无统计学意义(P>0.05);NAFLD患者SF和miR-122水平随着NAFLD患者病情的加重而呈上升趋势,不同程度患者之间差异有统计学意义(P<0.05),而25-OH Vit D水平呈下降趋势,不同程度患者之间差异有统计学意义(P<0.05);NAFLD患者血清中SF和miR-122水平与NAFLD病情严重程度呈正相关(r=0.592、0.639,P<0.05),而25-OH Vit D水平与NAFLD病情严重程度呈负相关(r=-0.617,P<0.05)。结论:NAFLD患者SF和miR-122水平明显升高,且与病情严重程度呈正相关,而25-OH Vit D水平明显下降,与病情严重程度呈负相关。因此,SF、25-OH Vit D及miR-122水平检测对NAFLD诊断及严重程度的鉴别诊断具有重要的意义,值得临床推广应用。
Objective:To investigate the expression and clinical significance of ferritin(SF),25 hydroxyl vitamin D[25-OH Vit D]and miR-122 in patients with different degrees of nonalcoholic fatty liver disease(NAFLD),so as to provide scientific basis for the diagnosis and severity of NAFLD.Method:From November 2017 to May 2019,270 patients diagnosed with NAFLD were selected as NAFLD group and 120 healthy patients were selected as HC group.The serum levels of SF,25-OH Vit D and miR-122 were detected respectively.Result:SF and miR-122 levels in NAFLD group were significantly higher than those in HC group,the difference was statistically significant(P<0.05),while the level of 25-OH Vit D were significantly lower than those in HC group,the difference was statistically significant(P<0.05).SF and 25-OH Vit D levels in male NAFLD patients were significantly higher than those in female NAFLD patients,the difference was statistically significant(P<0.05),but there was no significant difference between two groups in the level of miR-122(P>0.05).The levels of SF and miR-122 in NAFLD patients increased with the aggravation of NAFLD patients’condition,with statistically significant differences among patients of different degrees(P<0.05),while the level of 25-OH Vit D decreased,with statistically significant differences among patients of different degrees(P<0.05).The levels of SF and miR-122 in NAFLD patients were positively correlated with the severity of NAFLD(r=0.592,0.639,P<0.05),while the level of 25-OH Vit D were negatively correlated with the severity of NAFLD(r=-0.617,P<0.05).Conclusion:The levels of SF and miR-122 in NAFLD patients are significantly increased and positively correlated with the severity of the disease,while the level of 25-OH Vit D is significantly decreased and negatively correlated with the s everity of the disease.Therefore,the detection of SF,25-OH Vit D and miR-122 levels are of great significance for the differential diagnosis of NAFLD diagnosis and severity,and worthy of clinical application.
