浅析自拟淮黄调经汤治疗肾虚痰湿型多囊卵巢综合征的效果及对中医证候、内分泌和糖脂代谢的影响

Analysis of the Efficacy of Self-made Huaihuang Tiaojing Decoction in the Treatment of Kidney Deficiency and Phlegm Dampness Type Polycystic Ovary Syndrome and Its Influence on TCM Syndrome,Endocrine and Glucose and Lipid Metabolism

目的:浅析自拟淮黄调经汤治疗肾虚痰湿型多囊卵巢综合征(polycystic ovarian syndrome,PCOS)的效果及对中医证候、内分泌和糖脂代谢的影响.方法:以2016年1月-2019年1月笔者所在医院门诊及住院收治的104例肾虚痰湿型PCOS患者为研究对象,采用随机数字表法将其分为对照组(n=52)和分析组(n=52).对照组给予二甲双胍片与炔雌醇环丙孕酮片联合治疗,分析组在对照组基础上加用自拟淮黄调经汤治疗.比较两组治疗前后中医证候积分、糖脂代谢、生殖激素、性激素及治疗效果.结果:治疗前两组中医证候积分和糖脂代谢指标比较,差异均无统计学意义(P>0.05);治疗后两组均明显优于治疗前,且分析组优于对照组(P<0.05).两组治疗前生殖激素和性激素各项指标比较,差异均无统计学意义(P>0.05);治疗后除FSH外,两组LH、E2、TC、AMH均较治疗前改善,且分析组优于对照组(P<0.05).分析组治疗总有效率为92.31%,明显高于对照组的71.15%,差异有统计学意义(P<0.05).结论:对肾虚痰湿型PCOS采用常规西药治疗后,加用自拟淮黄调经汤治疗,可改善患者中医证候和糖脂代谢水平,以及生殖激素和性激素状况,相较于常规西药治疗方式,应用效果明显.

Objective:To analyze the efficacy of self-made Huaihuang Tiaojing Decoction in the treatment of kidney deficiency and phlegm dampness type polycystic ovary syndrome(PCOS)and its effects on TCM syndromes,endocrine and glucose and lipid metabolism.Method:From January 2016 to January 2019,104 patients with kidney deficiency and phlegm dampness type PCOS admitted to the outpatient and inpatient treatment of our hospital were taken as the research objects.They were divided into the control group(n=52)and the analysis group(n=52)by the random number table method.The control group was treated with Metformin Tablets combined with Ethinylestradiol Cyproterone Tablets.The analysis group was treated with self-made Huaihuang Tiaojing Decoction on the basis of the control group.The TCM syndrome score,glucose and lipid metabolism,reproductive hormone,sex hormone and therapeutic effect of the two groups before and after treatment were compared.Result:Before treatment,there were no significant differences in TCM syndrome score and glucose and lipid metabolism indexes between the two groups(P>0.05).After treatment,both groups were significantly better than before treatment,and the analysis group was better than the control group(P<0.05).There were no significant differences in the indexes of reproductive hormones and sex hormones between the two groups before treatment(P>0.05).After treatment,except for FSH,LH,E2,TC and AMH in the two groups were improved compared with those before treatment,and the analysis group was better than the control group(P<0.05).The total effective rate in the analysis group was 92.31%,which was significantly higher than 71.15% in the control group,and the difference was statistically significant(P<0.05).Conclusion:After conventional western medicine treatment for kidney deficiency and phlegm dampness type PCOS,the treatment with self-made Huaihuang Tiaojing Decoction can improve the TCM syndrome and glucose and lipid metabolism level of patients,and the status of reproductive hormones and sex hormones.Compared with conventional western medicine treatment,the treatment effect is obvious.
