摘要
目的:讨论高粘度骨水泥椎体成形术治疗骨质疏松性椎体压缩骨折的椎体高度恢复情况及相关影响因素.方法:于2015年1月-2016年12月对笔者所在医院68例骨质疏松性椎体压缩骨折患者作为研究对象,以骨水泥注入量是否超过4 ml分为甲1组29例(等于或未超过)及甲2组39例(超过);以骨水泥弥散率是否超过15%分为乙1组27例(等于或未超过)及乙2组41例(超过),所有患者均接受高粘度骨水泥椎体成形术,而后对不同组别患者手术前后骨折椎体高度、前缘高度与骨水泥注入量及骨水泥弥散程度之间的关系进行分析.结果:甲1组及甲2组患者术前的骨折椎体高度差异无统计学意义(P>0.05);甲2组患者术后1周的骨折椎体高度均明显高于甲1组,差异有统计学意义(P<0.05).乙1及乙2两组患者术前的骨折椎体高度差异无统计学意义(P>0.05);乙2组患者术后1周的骨折椎体高度均明显高于乙1组,差异有统计学意义(P<0.05).结论:骨水泥注入量及泥弥散重程度均可影响高粘度骨水泥椎体成形术后患者椎体高度的恢复,所以,实施该手术期间应保障充足的骨水泥量及高骨水泥弥散程度.
Objective:To discuss the recovery of vertebral height and related influencing factors in the treatment of osteoporotic vertebral compression fracture with high viscosity bone cement vertebroplasty.Method:From January 2015 to December 2016,68 patients with osteoporotic vertebral compression fracture in the hospital of the author were taken as the research objects.According to whether the injection amount of bone cement exceeded 4 ml,they were divided into 29 cases(equal or not exceeding)in group A 1 and 39 cases(exceeding)in group A 2.With bone cement dispersion rate was more than 15%can be divided into group B 1 of 27 cases(equal to or more than)and group B 2 of 41 cases(more than).All patients underwent high viscosity bone cement vertebroplasty,and the relationship between the height of the fractured vertebra and the height of the leading edge before and after surgery and the amount of bone cement injected and the degree of diffusion of bone cement were analyzed.Result:There was no significant difference in the height of fractured vertebral body between the group A 1 and group A 2 before surgery(P>0.05).The height of fractured vertebral bodies 1 week after surgery in group A 2 was significantly higher than that in group A 1,with statistically significant difference(P<0.05).There was no significant difference in the height of fractured vertebral body between the group B 1 and group B 2 before surgery(P>0.05).The fracture vertebral body height 1 week after surgery in group B 2 was significantly higher than that in group B 1,and the difference was statistically significant(P<0.05).Conclusion:Both the amount of bone cement injection and the dispersion weight of mud can affect the recovery of vertebral body height in patients with high viscosity bone cement vertebroplasty.Therefore,sufficient bone cement content and the dispersion degree of high bone cement should be guaranteed during the operation.
