老年人慢性心力衰竭与骨质疏松的相关性研究 被引量:2

摘要 目的:探讨老年人慢性心力衰竭(CHF)与骨质疏松(OP)之间的关系,为老年人慢性心力衰竭患者的临床诊治提供参考。方法:选取老年住院患者165例为研究对象,其中CHF组76例,非CHF组89例,对比两组患者OP发生率、BMD、PTH等指标;CHF组根据NYHA分级分为Ⅱ级组29例、Ⅲ级组28例、Ⅳ级组19例3组,对比3个亚组之间iPTH、BMD的差异。结果:CHF组患者腰椎及髋部BMD均低于非CHF组,而OP发病率及iPTH数值高于非CHF组,差异有统计学意义(P<0.05)。对CHF组进行亚组分析,在腰椎及髋部BMD的对比方面,心功能IV级组与Ⅱ级组、Ⅲ级组差异有统计学意义(P<0.05),Ⅱ级组与Ⅲ级组之间差异无统计学意义(P>0.05);在iPTH指标对比方面,心功能Ⅳ级组显著高于Ⅱ级组,差异有统计学意义(P<0.05),而Ⅱ级组与Ⅲ级组、Ⅲ级组与Ⅳ级组之间差异无统计学意义(P>0.05)。结论:老年人CHF与OP之间存在紧密的联系,CHF患者更容易发生OP,且随着心力衰竭的进展骨密度下降,骨折风险增加。
