柴胡舒肝散合桃红四物汤加减联合奥卡西平治疗带状疱疹后遗神经痛疗效观察

Observation on the Curative Effect of using Chaihu Shugan Powde r Combined with Modified Taohong Siwu Decoction and Oxcarbazepine in Treating Postherpetic Neuralgia

目的:观察柴胡舒肝散合桃红四物汤加减联合奥卡西平治疗带状疱疹后遗神经痛(PHN)的临床疗效。方法:将42例PHN患者随机分为对照组和治疗组,每组21例,两组患者均予以口服营养神经药物等基础治疗,在此基础上,对照组口服奥卡西平片0.3g/次,2次/d;治疗组在对照组的基础上给予柴胡舒肝散合桃红四物汤加减治疗。两组患者均治疗4周。观察两组患者在治疗前后的疼痛程度(VAS评分)改变、焦虑情况(SAS评分)、抑郁情况(SDS评分)以及睡眠质量(AIS评分)的改善情况。结果:对照组与治疗组治疗总有效率分别是76.19%和95.23%,差异有统计学意义(P<0.05);治疗后治疗组患者的VAS评分、SAS评分、AIS评分均显著低于对照组(P<0.05);治疗组与对照组相比,两组的SDS评分虽都有所下降,但差异无统计学意义(P>0.05)。结论:柴胡舒肝散合桃红四物汤加减联合奥卡西平对PHN疗效显著,无明显副作用,不仅能够有效地减轻患者疼痛症状,还能改善患者的焦虑及失眠情况,从而改善患者心理状态及提高生活质量,具有临床应用价值。

Objective:To observe the clinical effect of Chaihushugansan combined with Taohong Siwu Decoction combined with oxcarbazepine in the treatment of postherpetic neuralgia(PHN).Methods:42 PHN patients were randomly divided into a control group and a treatment group,with 21 patients in each group.The two groups were given basic treatments such as oral nutrition and neuropharmaceuticals.Based on this,the control group received oxcarbazepine 0.3 g/time.,2 times/d;the treatment group was given Chaihu Shugan Powder and Taohong Siwu Decoction on the basis of the control group.Patients in both groups were treated for 4 weeks.Observe the changes in pain(VAS score),anxiety(SAS score),depression(SDS score),and sleep quality(AIS score)before and after treatment in the two groups.Results:The total effective rates of the control group and the treatment group were 76.19%and 95.23%,respectively,and the differences were statistically significant(P<0.05).The VAS score,SAS score,and AIS score of the treatment group were significantly lower than those of the control group after treatment(P<0.05);Compared with the control group,the SDS scores of the two groups decreased,but the difference was not statistically significant(P>0.05).Conclusion:Chaihu Shugan Sanhe Taohong Siwu Decoction combined with oxcarbazepine has a significant effect on PHN without significant side effects.It can not only effectively reduce the pain symptoms of patients,but also improve the patient’s anxiety and insomnia,thereby improving the psychological state And improve the quality of life,it has clinical application value.
