黄芪建中汤加减治疗慢性浅表性胃炎所致慢性萎缩性胃炎的临床效果观察

Clinical observation of Huangqi Jianzhong decoction on chronic atrophic gastritis caused by chronic superficial gastritis

目的探讨黄芪建中汤辨证加减治疗慢性浅表性胃炎迁延所致慢性萎缩性胃炎的临床疗效。方法 90例慢性浅表性胃炎所致慢性萎缩性胃炎患者,根据治疗方法不同分为对照组和观察组,每组45例。对照组患者接受常规西药治疗,观察组患者接受黄芪建中汤辨证加减治疗。对比两组患者临床疗效、治疗前后中医症候和体征积分、药物不良反应发生情况、复发情况。结果观察组患者总有效率为97.78%,高于对照组的75.56%,差异具有统计学意义(P<0.05)。治疗前,两组患者手足不温、纳呆便溏、乏力、嗳气反酸、胃胀、胃痛积分对比差异无统计学意义(P>0.05);治疗后,两组患者手足不温、纳呆便溏、乏力、嗳气反酸、胃胀、胃痛积分均较本组治疗前明显降低,且观察组降低程度大于对照组,差异具有统计学意义(P<0.05)。治疗期间两组患者均没有出现明显药物不良反应,观察组复发率2.22%明显低于对照组的15.56%,差异具有统计学意义(P<0.05)。结论黄芪建中汤辨证加减治疗慢性浅表性胃炎迁延所致慢性萎缩性胃炎的临床疗效满意,值得推广应用。

Objective To discuss the clinical efficacy of Huangqi Jianzhong decoction on chronic atrophic gastritis caused by chronic superficial gastritis. Methods A total of 90 patients with chronic atrophic gastritis caused by chronic superficial gastritis were divided into control group and observation group by different treatment methods, with 45 cases in each group. The control group was treated by conventional Western medicine, and the observation group was treated by dialectical addition and subtraction of Huangqi Jianzhong decoction. The clinical efficacy, traditional Chinese medicine symptoms and signs score before and after treatment and occurrence of adverse drug reactions were compared between the two groups. Results The total effective rate was 97.78% in the observation group, which was higher than 75.56% in the control group, and the difference was statistically significant(P<0.05). Before treatment, there was no statistically significant difference in lack of warmth in the extremities, loose stools, fatigue, belching and acid regurgitation, flatulence and stomachache integral between the two groups(P>0.05). After treatment, the lack of warmth in the extremities, loose stools, fatigue, belching and acid regurgitation, flatulence and stomachache integral in the two groups was obviously lower than those before treatment, and the reduction in the observation group is greater than that in the control group. Their difference was statistically significant(P<0.05). No significant adverse drug reactions occurred in both groups of patients during treatment. The recurrence rate 2.22% in the observation group was obviously lower than 15.56% in the control group, and the difference was statistically significant(P<0.05). Conclusion Dialectical addition and subtraction of Huangqi Jianzhong decoction shows satisfactory clinical effect on chronic atrophic gastritis caused by chronic superficial gastritis, and it is worthy of promotion and application.
