摘要
目的通过超声造影观察颈动脉斑块新生血管的生成情况来预测斑块的稳定性,评估其与脑梗死事件发生的相关性。方法选取颈动脉超声检查有颈动脉斑块的患者116例分为脑梗死组61例和无脑梗死组(对照组)55例,除常规超声检查外,根据斑块风险评分行超声造影检查,记录2组患者的颈动脉斑块新生血管的显影程度,并分析不同显影程度分级斑块患者的脑梗死发生率,将斑块不同显影程度分级与脑梗死发生率进行相关性分析。结果脑梗死组斑块显影程度以Ⅲ级和Ⅳ级为主,占67.22%。非脑梗死组斑块显影程度以Ⅰ级和Ⅱ级为主,占80.00%。斑块新生血管不同显影程度分级患者的脑梗死发生率比较,差异有统计学意义(P<0.05),斑块新生血管显影程度分级与脑梗死发生率呈正相关。结论超声造影检查能有效评估颈动脉斑块的稳定性与脑梗死发生间的关系,且无创、简便,值得临床推广。
Objective To investigate the correlation between the stability of carotid plaques and cerebral infarction by examining the degree of neovascularization of carotid plaques by contrast-enhanced ultrasound.Methods A total of 116 patients with carotid plaque were examined by carotid ultrasound,including 61 cases of cerebral infarction and 55 cases of non-cerebral infarction group(control group),in addition toconventional ultrasound examination,according to the branch contrast-enhanced ultrasound examination of plaque risk assessment,the carotid plaque ultrasound contrast grade of the two groups of patients was recorded,The incidence of cerebral infarction in patients with different degree of ultrasound development was analyzed,and the correlation between the grade of plaque development and the incidence of cerebral infarction was analyzed.Results In the cerebral infarction group,the grade of plaque development was mainly gradeⅢandⅣ,and the proportion was 67.22%.in the non-infarction group,the grade of plaque development was mainly gradeⅠandⅡ,and the proportion was 80.00%.a comparison of the incidence of cerebral infarction in patients with the degree of neovascularization of carotid plaques,the difference was significant(P<0.05),the degree of neovascularization of plaques was positively correlated with the incidence of cerebral infarction.Conclusion Contrast-enhanced ultrasound can effectively evaluate the relationship between the stability of carotid plaques and the occurrence of cerebral infarction,and non-invasive,simple,worthy of clinical application and promotion.
