内镜黏膜切除术治疗结直肠息肉的临床疗效及不良反应发生率影响评价

Evaluation of clinical efficacy and incidence of adverse reactions in endoscopic mucosal resection for colorectal polyps

目的探析内镜下黏膜切除术(EMR)治疗结直肠息肉的疗效以及不良反应。方法选取2018年3月至2019年10月于本院诊治的结直肠息肉患者60例为研究对象,随机分为对照组和观察组,每组30例。对照组患者行内镜下高频电凝电切术治疗,观察组患者行内镜下黏膜切除术治疗,对比两组患者手术指标和术后不良反应发生率等。结果两组手术时间比较差异无统计学意义;两组胃肠道功能恢复时间、肛门直肠恢复时间对比差异均有统计学意义(P<0.001);观察组息肉完全切除率为92.31%,高于对照组的77.78%(P<0.05);观察组的术后不良反应发生率为3.33%、随访6个月息肉复发率为3.33%,均低于对照组的23.33%、20.0%(P<0.05)。结论内镜下的黏膜切除术在结直肠息肉患者中应用效果确切,能彻底切除息肉,预防息肉复发,且手术创伤小,术后不良反应少。

Objective To explore the efficacy and adverse endoscopic mucosectomy reactions(EMR)in the treatment of colorectal polyps.Methods 60 cases of colorectal polyps diagnosed and treated in our hospital from March 2018 to October 2019 were randomly divided into two control group and observation group,with 30 cases in each group.Patients in the control group were treated with endoscopic high-frequency electro coagulation,patients in the observation group were treated with endoscopic mucosal resection,compare the surgical indicators and the incidence of postoperative adverse reactions between the two groups.Results There was no significant difference in the operation time between the two groups.The difference of recovery time of gastrointestinal function and anorectal function between the two groups was statistically significant(P<0.001).The complete resection rate of polyps in the observation group was 92.31%higher than that in the control group of 77.78%(P<0.05).The incidence of postoperative adverse reactions in the observation group was 3.33%and the recurrence rate of polyps at the follow-up of 6 months was 3.33%,which was lower than the control group of 23.33%,20.0%(P<0.05).Conclusion Endoscopic mucosal resection is effective in colorectal polyps.It can completely remove polyps and prevent recurrence of polyps.
