调神法针刺治疗慢性前列腺炎/慢性骨盆疼痛综合症临床研究

Clinical Study of Tiaoshen Acupuncture in Treating Chronic Prostatitis/Chronic Pelvic Pain Syndrome

目的:比较针灸与药物治疗慢性前列腺炎/慢性骨盆疼痛综合症(CP/CPPS)的疗效差异。方法:采用随机对照实验设计,将符合纳入标准的CP/CPPS70例临床患者用随机数表法分为观察组与对照组,观察组脱落2例,剩余33例;对照组脱落1例,剩余34例。观察组在调神法针刺的基础上配合常规针刺治疗,每日1次,1周6次;对照组口服常规药物治疗,两组均治疗4周。观察两组治疗前、后的慢性前列腺炎症状指数(NIH-CPSI)评分、前列腺液(EPS)中白细胞计数及抑郁量表(HAMD)评分。结果:治疗后观察组总有效率为93.9%(31/33),高于对照组的85.3%(29/34),差异有统计学意义(P<0.05);两组患者治疗后NIH-CPSI评分、EPS中白细胞计数、HAMD评分较治疗前均下降(均P<0.05),两组患者治疗后上述指标组间比较,差异有统计学意义(P<0.05),两组患者治疗前后NIH-CPSI评分、EPS中白细胞计数、HAMD评分差值组间比较,差异均有统计学意义(P<0.05)。结论:调神法针刺治疗CP/CPPS效果显著,可降低EPS中白细胞数量,缓解炎症,有效改善患者抑郁情绪,疗效优于常规药物治疗。

Objective:To compare the effect difference of acupuncture and medication in treating chronic prostatitis/chronic pelvic pain syndrome(CP/CPPS).Methods:A randomized controlled trial(RCT)was designed to include 70 patients with CP/CPPS who met the inclusion criteria,the patients were randomly divided into the observation group(n=33,two cases missing)and the control group(n=34,one case missing).The observation group was treated with Tiaoshen acupuncture based on conventional acupuncture(once a day,6 times a week).The control group was treated with conventional medication.Both groups were treated for four weeks.The NIH-CPSI scores of chronic prostatic inflammatory index,white blood cell count in expressed prostatic secretions(EPS)and HAMD scores of depression scale were observed before and after the treatment in the two groups.Results:After the treatment,the total effective rate of the observation group was 93.9%(31/33),which was significantly higher than 85.3%(29/34)of the control group(P<0.05).NIH-CPSI scores,white blood cell count of EPS and HAMD scores decreased significantly after the treatment compared to those before the treatment(P<0.05);of which there were statistical differences between the two groups(P<0.05);the differences in the improvements of above indexes were also significant between the two groups(P<0.05).Conclusion:Tiaoshen acupuncture has a significant effect on CP/CPPS,it can reduce the amount of leukocytes in EPS,alleviate inflammation and effectively improve patients'depression,which is better than conventional medication.
