摘要
目的探讨碱性磷酸酶、同型半胱氨酸和游离脂肪酸对急性脑梗死患者预后的评估价值。方法选取435例急性脑梗死患者作为研究对象,根据患者脑梗死发作后第30天Rankin量表评分的结果将患者分为预后良好组和预后不良组,比较分析两组患者的临床资料和各项实验室指标。结果多变量Logistic模型得出碱性磷酸酶(alkaline phosphatase,ALP)、同型半胱氨酸(homocysteine,Hcy)、游离脂肪酸(free fatty acid,FFA)与急性脑梗死患者的预后不良密切相关,3种指标的优势比和95%的可信区间分别为:ALP(1.091;1.058~1.124),Hcy(0.923;0.872~0.978),FFA(0.930,0.889~0.974)。结论碱性磷酸酶、同型半胱氨酸和游离脂肪酸的检测对急性脑梗死的预后评估有重要意义,值得临床参考应用。
Objective To investigate the clinical values of alkaline phosphatase,homocysteine and free fatty acid in the prognosis of acute cerebral infarction.Methods Four hundred and thirty-five patients with acute cerebral infarction were enrolled in our study.The patients were divided into 2 groups according to modified Rankin Scale(mRS)scores evaluated on the 30th day after onset of acute cerebral infarction,and we analyzed the clinical data and laboratory indexes of the two groups.Results Multivariate logistic analysis confirmed that alkaline phosphatase,homocysteine and free fatty acid were significantly related to poor prognosis in acute cerebral infarction.The odds ratio and 95%confidence interval of the three indicators were as following:alkaline phosphatase(1.091;95%CI:1.058-1.124),homocysteine(0.923;95%CI:0.872-0.978),and free fatty acid(0.930;95%CI:0.889-0.974).Conclusion The levels of alkaline phosphatase,homocysteine and free fatty acid have certain clinical significances for the prognosis evaluation of acute cerebral infarction,suggesting a potential clinical reference application.
