摘要
目的分析慢性自发性荨麻疹(CSU)患者血清白介素-35(IL-35)、转化生长因子-β1(TGF-β1)、总免疫球蛋白E(IgE)水平变化及其与瘙痒程度的关系。方法回顾性研究,收集CSU患者83例及同期门诊健康体检查者40例,均测定IL-35、TGF-β1、总IgE水平,CSU患者依据视觉模拟评分(VAS)评定瘙痒程度,记录每日瘙痒时间,比较不同瘙痒程度及瘙痒时间CSU患者血清上述因子水平的差异,分析各因子与瘙痒程度及时间的关系。结果CSU组血清IL-35低于健康对照组,TGF-β1及总IgE水平均高于健康对照组(P<0.05);不同瘙痒程度评分、每日不同瘙痒时间CSU患者血清IL-35、TGF-β1、总IgE水平比较差异皆有统计学意义(P<0.05),IL-35水平比较:轻度组>中度组>重度组(P<0.05)、低于30min组>30~120min组>超过120min组(P<0.05);TGF-β、总IgE水平比较:轻度组<中度组<重度组(P<0.05)、低于30min组<30~120min组<超过120min组(P<0.05);IL-35与瘙痒程度评分、瘙痒时间呈负相关(P<0.05),TGF-β1、总IgE与瘙痒程度评分及瘙痒时间均呈正相关(P<0.05)。结论CSU患者以低IL-35,高TGF-β1、总IgE水平为特点,随瘙痒程度的增加及瘙痒时间的延长,IL-35持续降低,TGF-β1、总IgE水平上升,调节细胞免疫、拮抗IgE或可能为CSU瘙痒症状控制的新靶点。
Objective To analyze the changes of serum interleukin-35(IL-35),transforming growth factor-β1(TGF-β)and total immunoglobulin E(IgE)levels in patients with chronic spontaneous urticaria(CSU)and their correlations with itching degrees.Methods A retrospective study was conducted with a cohort of 83 patients with CSU and 40 health people.The levels of IL-35,TGF-β1 and total IgE in all subjects were determined.The itching degree of patients with CSU was assessed with the visual analogue scale(VAS).The duration of itching was recorded every day.Serum levels of above-mentioned factors in patients with CSU and degrees of itching and itching duration were compared,and the relationship between each factor and the degree and duration of itching was evaluated.Results The serum IL-35 level in the CSU group was lower than that in the healthy control group,while levels of TGF-β1 and total IgE were higher than the healthy control group(P<0.05).There were significant differences in serum IL-35,TGF-β1 and total IgE levels within CSU patients with different scores of itching degree and different daily itching duration(P<0.05).IL-35 level was the highest in the mild group,followed by the moderate group and severe group(P<0.05),and was the highest in the shorter-than-30 min group,followed by the 30-120 min group and the over-120 min group(P<0.05).TGF-βand total IgE levels were the lowest in the mild group,followed by the moderate group and severe group(P<0.05),and were the lowest in the shorter-than-30 min group,followed by the 30-120 min group and overr-120 min group(P<0.05).IL-35 was negatively correlated with the score of itching degree and itching duration(P<0.05),while TGF-β1 and total IgE were positively correlated with the score of itching degree and itching duration(P<0.05).Conclusion CSU patients are characterized by low IL-35 level,high TGF-β1 and total IgE levels.With the increase of itching degree and prolongation of itching duration,IL-35 continuously decreases while TGF-β1 and total IgE levels increase.Regulating cellular immunity and antagonizing IgE could be a new target for itching control.
