急性心肌梗死患者个案管理模式研究

The effect of case management model in patients with acute myocardial infarction

目的探讨个案管理模式在急性心肌梗死出院患者随访中的应用效果。方法将64例急性心肌梗死出院患者分为试验组31例采用个案管理模式进行随访,对照组33例接受常规随访。结果实施个案管理模式管理后12个月,试验组患者非计划性重返率低于对照组(P<0.01),试验组患者左心室射血分数、左心室舒张末期内径的稳定性均优于对照组(P<0.01),试验组患者自我效能提高(P<0.01)。结论对急性心肌梗死出院患者实施个案管理能够降低非计划性重返率,稳定患者的心功能,提高患者自我效能。

Objective To investigate the application effect of case management mode in the follow-up of discharged patients with acute myocardial infarction.Methods A total of 64 patients with acute myocardial infarction discharged from hospital were divided into the experimental group,in which 31 patients were followed up by case management mode,and the control group,in which 33 patients were followed up by routine mode.Results After 12 months of follow-up with the implementation of the case management model,the unplanned return rate of patients in the experimental group was lower than that in the control group(P<0.01).Meanwhile the stability of left ventricular ejection fraction and left ventricular end-diastolic diameter in the experimental group were better than those in the control group(P<0.01),and the self-efficacy of patients in the experimental group was improved(P<0.01).Conclusion Case management of discharged patients with acute myocardial infarction can reduce the rate of unplanned return,stabilize the cardiac function of patients,and improve the self-efficacy of patients.
