青盲源流考

Investigation on origin and development of Qingmang

本文通过查阅古代文献,从病名、病因病机、治法方面对青盲源流进行考证。梳理青盲的病名沿革,探究青盲的学术脉络和诊治规律,剖析治法,以期指导临床。“青盲”一词最早见于《神农本草经》,后沿用至今。青盲病位在目,以目无所见为主症。青盲的病因病机多端:情志失常、气滞血瘀;热气拂郁、通光失灵;病邪乘脏、气血亏虚;肝肾亏虚、目窍失养;脾肾阳虚、目失温养;他病传变、金兽所伤;胎中受邪、小儿致盲等均可导致该病的发生。青盲之治疗,当依病机,选用内治、外治法,随证治之。本病难治,预后不良,若为他病传变,当图早治,以减轻患者临床症状为主。

By consulting the ancient literature,this paper made a textual research on the source of Qingmang from the aspects of disease name,etiology,pathogenesis and treatment.In order to guide the clinical practice,the evolution of the disease name was reviewed,the academic context,diagnosis and treatment rules were explored,and the treatment methods were analyzed in this paper.The term Qingmang first appeared in Shen Nong Ben Cao Jing(Shennong’s Herbal)and had been in use ever since.The location of the disease is in the eye,and the main symptom is blindness.The etiology and pathogenesis include moodiness,qi stagnation and blood stasis,heat evil disturbance,light transmission obstruction,evil overwhelms viscera,dual deficiency of qi and blood,liver and kidney deficiency,malnutrition of the eyes,spleen and kidney yang deficiency,mal-warming nurturing of the eyes,transmutation of relate diseases,traumatic injury damaging ocular collaterals,fetal pathogen,and so on.The treatment of Qingmang should be based on the pathogenesis,and internal treatment and external treatment should be used in combination,along with syndrome treatment.The disease is difficult to treat,and the prognosis is poor.If it is the transmutation of other diseases,it should be treated as early as possible in order to alleviate the clinical symptoms.
