腹腔镜阑尾切除术与开腹阑尾切除治疗急性阑尾炎的临床效果比较

Comparison the clinical effect of laparoscopic appendectomy and open appendectomy in the treatment of acute appendicitis

目的 比较腹腔镜阑尾切除术与开腹阑尾切除治疗急性阑尾炎的临床效果.方法 选取2017年2月~2018年2月我院收治的100例急性阑尾炎患者作为研究对象,按照随机数字表法分为观察组(50例)和对照组(50例).观察组采用腹腔镜阑尾切除术治疗,对照组采用开腹阑尾切除治疗.比较两组的手术指标、手术前后视觉模拟量表(VAS)评分及并发症总发生率.结果 观察组的手术用时、肛门排气时间及住院时间短于对照组,出血量少于对照组,差异均有统计学意义(P<0.05).两组术前的VAS评分比较,差异无统计学意义(P>0.05);观察组术后6、12、24 h的VAS评分低于对照组,差异均有统计学意义(P<0.05).观察组的并发症总发生率低于对照组,差异有统计学意义(P<0.05).结论 腹腔镜阑尾切除术应用于急性阑尾炎,能缩短手术用时、肛门排气时间及住院时间,减少出血量,缓解患者的术后疼痛程度,降低并发症发生率,值得临床推广应用.

Objective To compare the clinical effect of laparoscopic appendectomy and open appendectomy in the treatment of acute appendicitis.Methods A total of 100 patients with acute appendicitis in our hospital from February 2017 to February 2018 were selected as the study objects,they were divided into observation group(50 cases)and control group(50 cases)according to the random number table method.The observation group was treated with laparoscopic appendectomy,the control group was treated with open appendectomy.Surgical indicators,visual analog scale(VAS)scores before and after surgery,and total complication rate were compared between the two groups.Results The operation time,anal exhaust time and hospitalization time of the observation group were shorter than those of the control group,the amount of bleeding was less than that of the control group,the differences were statistically significant(P<0.05).There was no significant difference in VAS score before surgery between the two groups(P>0.05).The VAS scores in the observation group at 6,12,and 24 hours after surgery were lower than those in the control group,the differences were statistically significant(P<0.05).The total incidence of complications in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion Laparoscopic appendectomy is applied to acute appendicitis,which can shorten the operation time,anal exhaust time and hospitalization time,reduce the amount of bleeding,relieve the patient′s postoperative pain,and reduce the incidence of complications.
