基于内容分析法的互操作性框架下医院信息系统集成现状

Descriptive Study of Hospital Information System Integration under the Framework of Interoperability Based on Content Analysis

目的借助互操作性框架了解医院信息系统集成的整体情况和发展趋势,为后期医院信息系统集成应用和研究提供参考。方法基于万方数据库和中国知网数据库,检索该数据库从1997年截至2019年10月的医院信息系统集成相关文献,利用文献计量学方法和内容分析法等进行梳理。结果共筛选出107篇文献,数据、服务、流程及业务等集成关注点的文献比例依次为65.42%、18.69%、35.51%、57.94%;技术障碍、语义障碍、结构障碍占比分别为79.44%、59.81%、48.60%;技术方法、语义方法、组织(结构)方法占比分别为63.55%、57.94%、68.22%,对分类结果进行信度检验,克朗巴哈α系数为0.844,内容分析评判达到可接受程度。结论结合互操作性框架的梳理,集成方案设计之初对于服务的关注应加强,集成障碍的研究中应该在医院内部集成的基础上关注医院间的组织兼容性。具体方法中需要明确技术、概念及组织层面的不兼容性并针对其形成可行性方案。

Objective To systematically analyze the overall situation and trend of hospital information system integration through the interoperability framework,and provide reference for the integration and application of hospital information system in the later stage.Methods Based on Wanfang data knowledge service platform and China knowledge network database,related literatures on hospital information system integration from 1997 to October 2019 were retrieved,and the bibliometric method and content analysis method were used for combing.Results A total of 107 articles were selected.The proportion of literatures with integration concerns about data,services,processes and business was 65.42%,18.69%,35.51%,and 57.94%,respectively.The proportion of the technological barrier,conceptual barrier,and organizational barrier accounted for 79.44%,59.81%,48.60%.The proportion of technological methods,conceptual methods,organizational methods accounted for 63.55%,57.94%,68.22%,respectively.The reliability of the classification results,Cronbach alpha coefficient was 0.844,and the content analysis judged to an acceptable level.Conclusion Combining with the interoperability framework,the attention to service should be strengthened at the beginning of the integration scheme design.In the research of integration barriers,the organizational compatibility between hospitals should be paid attention to on the basis of hospital internal integration.In the specific method,it is necessary to clarify technical,conceptual and organizational incompatibility and form a feasible solution for it.
