摘要
Barrett食管与胃食管反流病密切相关,近年来反流导致的黏膜损伤与Barrett食管的关系受到越来越多的重视,其中,DNA损伤及修复过程直接参与了反流导致的黏膜损伤修复,并可能与Barrett食管及食管腺癌的发生、发展密切相关。目前,已有临床试验证实胃食管反流可直接导致食管黏膜的DNA损伤修复,部分参与DNA损伤修复的蛋白能调控食管上皮细胞肠化生。因此,从胃食管反流病产生的DNA损伤应答来深入研究Barrett食管的发生机制,从分子水平防治Barrett食管是临床研究关注的问题。本文就近年胃食管反流病和Barrett食管的证据及与DNA损伤应答机制进展进行综述。
Barrett's esophagus is closely associated with gastroesophageal reflux disease(GERD).In recent years,the relationship between reflux-induced mucosa damage and Barrett's esophagus has been paid more and more attention,in which,DNA damage and repair process are directly involved in the repair of mucosa damage caused by reflux.It may be closely related to the development of Barrett's esophagus and esophageal adenocarcinoma.At present,there have been accurate clinical and experimental evidence that GERD can directly lead to DNA damage and repair in esophagus mucosa,and part of the DNA repair protein could regulate the intestinal growth of esophagus epithelial cell lines.Therefore,we reviewed the recent studies of the occurrence mechanism of Barrett's esophagus and GERD from the aspect of the DNA damage responses,to prevention and treatment of Barrett's esophagus in the molecular level.The molecular mechanism of DNA damage response that occur during the transformation of Barrett's esophagus is reviewed and discussed.
