平均血小板体积与视网膜中央动脉阻塞的关系探讨

目的探讨平均血小板体积(MPV)与视网膜中央动脉阻塞(CRAO)的关系。方法将2017年1月~2019年1月在本院诊断为CRAO的96例患者纳入CRAO组,另按1:1比例选取同期在本院接受健康体检的96例健康志愿者作为对照组;比较两组一般临床资料、MPV水平,先行多因素Logistic回归分析CRAO的独立危险因素,并绘制受试者工作特征(ROC)曲线分析MPV对CRAO的诊断价值。结果两组性别、身体质量指数(BMI)、年龄、空腹血糖、肾小球滤过率(eGFR)、血小板计数(PLT)、纤维蛋白原(Fbg)比较差异无统计学意义(P>0.05);但观察组高血压、高脂血症、吸烟史、白细胞计数(WBC)、MPV均显著高于对照组,差异有统计学意义(P<0.05);多因素Logistic回归分析,MPV与高血压、高脂血症、WBC均是CRAO的独立危险因素(P<0.05);经ROC曲线分析,曲线下面积值(AUC)为0.652(0.575~0.729),约登指数为0.281,以8.716为临界值,其诊断CRAO的敏感度、特异度分别为77.10%、51.00%。结论MPV与高血压、高脂血症、WBC均是CRAO的独立危险因素,作为诊断CRAO的临床指标虽具一定预测价值。

Objective To investigate the relationship between mean platelet volume(MPV)and central retinal artery occlusion(CRAO).Methods 96 patients diagnosed as CRAO in our hospital from January 2017 to January 2019 were included in the CRAO group,and 96 healthy volunteers who received physical examination in our hospital at the same time were selected as the control group according to the ratio of 1:1.The general clinical data and MPV level of the two groups were compared,and the independent risk factors of CRAO were analyzed by multi factor Logistic regression first,and the receiver operating character was drawn The diagnostic value of MPV to CRAO was analyzed by the curve of istic,(ROC).Results There was no significant difference in sex,body mass index(BMI),age,fasting blood glucose,estimated glomerular filtration rate(EGFR),platelet count(PLT),fibrinogen(Fbg)between the two groups(P>0.05),but there was no significant difference in hypertension,hyperlipidemia,smoking history,white blood cell count between the two groups(P>0.05)White blood cell count(WBC)and MPV were significantly higher than those of the control group(P<0.05);multivariate logistic regression analysis showed that MPV,hypertension,hyperlipidemia and WBC were independent risk factors of CRAO(P<0.05);by ROC curve analysis,area under the ROC curve(AUC)was 0.652(0.575-0.729),and yoden index was 0.281,The sensitivity and specificity of 8.716 were 77.10%and 51.00%,respectively.Conclusion MPV,hypertension,hyperlipidemia and WBC are independent risk factors of CRAO.Although they have certain predictive value as clinical indicators of CRAO.
