中药膏方治疗产后缺乳的临床对照观察

Clinical control observation of Chinese herbal ointment in treatingmaternal lack of lactation

目的:研究中药膏方治疗产后缺乳的临床疗效。方法:缺乳产妇378例,均为高密市人民医院2015年4月-2019年1月就诊病例,采用随机数字表法分为治疗组和对照组,治疗组198例,对照组180例,两组患者年龄、身体质量、血压、泌乳不足病程,以及血红蛋白、白蛋白等比较无统计学意义(P>0.05)。分别给予中药膏方治疗,5 d为1个疗程,观察产妇治疗前后临床症状、乳房胀度、泌乳量、新生儿体质量,检测产妇血清泌乳素、雌二醇、孕酮浓度等指标。结果:治疗2个疗程后,治疗组总有效率89.4%(177/198),对照组总有效率68.9%(124/180)。两组产妇治疗后乳房胀度、泌乳量与治疗前比较均有改善,治疗组改善程度优于对照组(P<0.05)。两组产妇治疗后血清泌乳素浓度均高于治疗前(P<0.05),雌二醇水平低于治疗前(P<0.05),孕酮水平治疗前后无变化(P>0.05)。结论:本研究中药膏剂对治疗产妇缺乳有理想的治疗效果,临床疗效呈量效正相关。

Objective:To study the clinical efficacy of traditional Chinese medicine ointment in the treatment of lactation deficiency and lactation.Methods:378 cases of lactation-deficient women were treated in our hospital from April 2015 to January 2019.They were divided the treatment group and the control group by random number table method,with 198 cases in treatment group and 180 cases in control group.There was no statistically significant difference in the age,body mass,blood pressure,duration of lactation,hemoglobin and albumin of the two groups.(P>0.05).The traditional Chinese medicine ointment was given for treatment.The course of 5 days as 1 course.The clinical symptoms,breast swelling,milk production,and neonatal body weight were observed before and after maternal treatment.The serum prolactin,estradiol and progesterone concentrations were measured.Results:After 2 courses of treatment,the total effective rate was 89.4%(177/198)in the treatment group and 68.9%(124/180)in the control group.The breast swelling and lactation volume of the two groups were improved after treatment,and the improvement of the treatment group was significantly better(P<0.05).The serum prolactin concentration in the two groups was higher(P<0.05),the estradiol level was lower(P<0.05),and the progesterone level did not change(P>0.05).Conclusions:Traditional Chinese medicine ointment has an ideal therapeutic effect on the treatment of maternal lactation deficiency,and the clinical effect is positively correlated.
